摘要
目的研究低分子肝素早期干预治疗胎儿生长受限(FGR)的临床效果。方法选取惠州市妇幼保健计划生育服务中心2016年3月-2018年10月期间产检确诊为胎儿生长受限的孕妇104例,采用随机数字法进行分组,其中52例进行常规基础治疗(对照组),另52例在前者基础上联合低分子肝素治疗(观察组)。比较两组治疗前后胎儿生长发育指标、胎盘血流动力学结果指标。结果治疗后,观察组胎儿的宫高、头围、腹围、双顶径每周增长幅度明显大于对照组(P<0.05),脐血流搏动指数、阻抗峰速比值、阻力指数均明显低于对照组(P<0.05),血栓性血管病明显少于对照组(P<0.05),差异均有统计学意义。结论低分子肝素治疗FGR的临床疗效较为理想,可有效改善胎盘血流动力学,促进胎儿宫内生长发育。
Objective To study the clinical effect of early intervention of low molecular weight heparin in the treatment of fetal growth restriction(FGR).Methods 104 pregnant women who were diagnosed as fetal growth restriction during the period from March 2016 to April 2017 in the Huizhou Maternal and child Health Family Planning Service Center were selected and grouped by random number method.Among them,52 patients underwent routine basic therapy(control group),and 52 patients were treated with low molecular weight heparin(observation group).The fetal growth and development indicators and placental pathological results were compared before and after treatment.The fetal growth and development indicators and placental hemodynamic outcome indicators were compared before and after treatment.Results After treatment,the uterine height,head circumference,abdominal circumference and biparietal diameter in the observation group were significantly higher than those in the control group(P<0.05).After treatment,the PI,S/D,RI of the fetus in the observation group was significantly lower than those in the control group(P<0.05).Fetal thrombotic vascular disease in the observation group was significantly less than that of the control group after treatment(P<0.05).Conclusion Low molecular weight heparin(LMWH)was effective in the treatment of FGR.It could effectively improve the placental hemodynamics and promote fetal intrauterine growth and development.
