解郁通腑汤配合腹部穴位按摩治疗功能性便秘(肝郁气滞型)的临床研究

Clinical study on Jieyu tongfu decoction combined with abdominal acupoint massage in the treatment of functional constipation(stagnation of liver-qi type)

目的探讨解郁通腑汤配合腹部穴位按摩治疗功能性便秘(肝郁气滞型)的临床效果。方法选择2018年1月—2020年1月本院收治的84例肝郁气滞型功能性便秘患者为研究对象,按随机数表法分为观察组与对照组两组,每组各42例。对照组采用腹部穴位按摩治疗,观察组在此基础上联合使用解郁通腑汤治疗。比较治疗2个月后,两组患者中医证候积分(肠鸣矢气、胸胁胀满、厚重窘迫、口干口苦)、生活质量,外周血T淋巴细胞亚群(CD3+、CD4+、CD8+、CD4+/CD8+)的变化。结果两组患者治疗后中医症候积分均较治疗前显著下降,且观察组患者中医症候积分均明显低于对照组,差异有统计学意义(P<0.05);两组患者治疗后生活质量各项评分均较治疗前显著降低,且观察组患者生活质量各项评分明显低于对照组,差异有统计学意义(P<0.05);两组患者治疗后CD3、CD8均较治疗前显著下降,CD4、CD4/CD8均较治疗前显著升高,差异有统计学意义(P<0.05);且观察组患者CD3、CD8均明显低于对照组,CD4、CD4/CD8均明显高于对照组,差异有统计学意义(P<0.05)。结论肝郁气滞型FC患者采用解郁通腑汤配合腹部穴位按摩治疗疗效较好,能够减轻症状,提高免疫力与生活质量。

Objective To explore the clinical effects of Jieyu tongfu decoction combined with abdominal acupoint massage on functional constipation(FC)(stagnation of liver-qi type).Methods 84 patients with FC of liver-qi stagnation type those who were admitted to our hospital from January 2018 to January 2020 were selected as the research subjects,and they were divided into observation group and control group according to the random number table method,with 42 cases in each group.Control group was given abdominal acupoint massage for treatment,and observation group was combined with Jieyu tongfu decoction on this basis.Two months after treatment,the changes of TCM syndromes scores(borborygmus and flatus,fullness of chest and hypochondrium,full-bodied distress,dry and bitter mouth),quality of life and peripheral blood T lymphocyte subsets(CD3+、CD4+、CD8+、CD4+/CD8+)were compared between the two groups.Results After 2 months of treatment,the TCM symptoms scores of the both groups were statistically decreased,and the scores of the observation group were statistically lower than the control group(P<0.05).After the treatment the scores of quality of life in both two groups were significantly decreased,and those of the observation group were lower than the control group(P<0.05).After the treatment,CD3 and CD8 in the two groups were significantly decreased compared with those before treatment,and the above indexes in observation group were significantly lower than those in control group(P<0.05).The CD4 and CD4/CD8 were significantly increased compared with those before treatment,and the two indexes in observation group were significantly higher than those in control group(P<0.05).Conclusions Jieyu tongfu decoction combined with abdominal acupoint massage has a good therapeutic efficacy in the treatment of patients with FC of liver-qi stagnation type,and it could relieve symptoms and improve immunity and quality of life.
