摄食训练联合改良康复吞咽操对脑卒中吞咽障碍患者康复效果及摄食-吞咽功能的影响

Effects of Feeding Training Combined with Improved Rehabilitation Swallowing Exercises on the Rehabilitation Effect and Feeding-swallowing Function of Stroke Patients with Dysphagia

目的分析摄食训练联合改良康复吞咽操对脑卒中吞咽障碍患者的康复效果及摄食-吞咽功能的影响。方法选择2020年1月至2021年1月南阳市中心医院收治的88例脑卒中吞咽障碍患者为研究对象,采用随机数字表法分为两组,对照组44例行常规干预,观察组44例予以摄食训练联合改良康复吞咽操,1个月后对比两组的康复效果、舌骨喉复合体的活动度、摄食-吞咽功能及生活质量。结果干预后,观察组的渗透-误吸等级量表(PAS)评分比对照组低,而功能性经口摄食量表(FOIS)评分则比对照组高(P<0.05);观察组的舌骨前移、上移活动度均比对照组高(P<0.05);观察组的藤岛摄食-吞咽功能评分为(8.66±1.12)分高于对照组的(7.57±1.01)分(P<0.05);观察组的吞咽障碍特异性生活质量量表(SWAI-QOL)评分是(180.19±18.76)分,比对照组的(165.29±20.30)分高(P<0.05)。结论摄食训练联合改良康复吞咽操在脑卒中吞咽障碍患者中的应用效果明显,有助于提升其康复效果及舌骨喉复合体的活动度,促进患者摄食-吞咽功能及生活质量进一步提高。

Objective To analyze the effect of feeding training combined with improved rehabilitation swallowing exercises on the rehabilitation effect and feeding-swallowing function of stroke patients with dysphagia.Methods A total of 88 stroke patients with dysphagia admitted to Nanyang Central Hospital between January 2020 and January 2021 were collected and divided into two groups according to the random number table method.The control group received routine intervention for 44 cases,and the observation group received dietary training combined with improvement for 44 cases.Rehabilitation of swallowing exercises,the rehabilitation effect,the activity of the hyoid-laryngopharyngeal complex,the feeding-swallowing function and the quality of life were compared between the two groups after 1 month.Results After intervention,the Penetration-Aspiration Rating Scale(PAS)score in the observation group was lower than that in the control group,while the Functional Oral Feeding Scale(FOIS)score was higher than that in the control group(P<0.05).The forward and upward mobility were higher than those in the control group(P<0.05);the Fujishima feeding-swallowing function score of the observation group was(8.66±1.12)points,which was higher than that of the control group(7.57±1.01)points(P<0.05).The swallowing disorder-specific quality of life scale(SWAI-QOL)score of the observation group was(180.19±18.76)points,which was higher than that of the control group(165.29±20.30)points(P<0.05).Conclusion The application effect of feeding training combined with improved rehabilitation swallowing exercise in stroke patients with dysphagia is significant,which helps to improve their rehabilitation effect and the mobility of the hyoid-laryngolaryngopharyngeal complex,and further improves the feeding-swallowing function and quality of life of patients.
