社区高血压和糖尿病管理中社区卫生定向服务模式的作用评估

Evaluation of the Role of Community Health Orientation Service Model in Community Hypertension and Diabetes Management

目的探讨以社区卫生定向服务模式综合了社区工作者用于社区高血压和糖尿病管理中的作用效果。方法2018年3月至2019年3月选取北京市朝阳区十八里店社区卫生服务中心高血压或糖尿病患者200例,随机分为对照组(100例)和观察组(100例),对照组采用常规护理,观察组在对照组的基础上采用社区卫生定向服务模式,观察两组患者血压和血糖变化情况以及患者对相关知识的知晓率和满意度情况。结果观察组患者护理后空腹血糖、餐后血糖、糖化血红蛋白、收缩压、舒张压均低于对照组(P<0.05)。观察组患者护理后对糖尿病和高血压知识知晓率〔96.0%(96/100)〕高于对照组〔81.0%(81/100)〕(χ^(2)=11.503,P<0.001)。观察组患者对社区定向服务模式的满意度〔98.0%(98/100)〕高于对照组〔82.0%(82/100)〕(χ^(2)=14.222,P<0.001)。结论社区卫生定向服务模式可以提高患者对糖尿病和高血压相关知识的知晓率,帮助患者疾病恢复,可以作为社区高血压和糖尿病的护理首选方式。

Objective Discusses the effect of community health oriented service model of community workers in community hypertension and diabetes management.Methods 200 patients with hypertension and diabetes were selected from March 2018 to March 2019,and randomly divided into the control group(100 cases)and the observation group(100 cases).The control group adopts conventional nursing care,and the observation group adopted community health orientation service based on the control group and observed the changes of hypertension and diabetes and the awareness and satisfaction of relevant knowledge.Results The fasting blood glucose,postprandial blood glucose,glycosylated hemoglobin,systolic blood pressure and diastolic blood pressure of the observation group after nursing were lower than those of the control group(P<0.05).The awareness rate of diabetes and hypertension knowledge of patients in the observation group after nursing〔96.0%(96/100)〕was higher than that of the control group〔81.0%(81/100)〕(χ^(2)=11.503,P<0.001).The satisfaction of patients in the observation group with the community-oriented service model〔98.0%(98/100)〕is higher than that of the control group〔82.0%(82/100)〕(χ^(2)=14.222,P<0.001).Conclusion Community health oriented service model can improve patient awareness of diabetes and hypertension related knowledge,and help patients with disease recovery,which can be used as the first way of care for community hypertension and diabetes.
