腹腔镜全胃切除胃癌根治术对进展期胃癌患者预后及术后并发症发生率的影响

Effect of Laparoscopic Total Gastrectomy and Radical Gastrectomy on Prognosis and Postoperative Complications in Patients with Advanced Gastric Cancer

目的腹腔镜全胃切除胃癌根治术对进展期胃癌患者预后及术后并发症发生率的影响分析。方法选择2018年1月至2021年2月涟水县人民医院中进行治疗的90例进展期胃癌患者为研究对象。以患者的治疗方式作为依据分为对照组(n=45)和观察组(n=45),对照组采取手术方式为常规开腹手术,观察组采取手术方式为腹腔镜全胃切除胃癌根治术,记录患者治疗效果、术后并发症情况以及围术期指标情况,并比较两组间有无差异。结果观察组患者临床总有效率高于对照组(P<0.05),并发症发生率低于对照组(P<0.05)。观察组患者手术时间长于对照组(P<0.05),手术切口长度、术中出血量、注射止痛药物次数、术后肛门排气时间、首次下床活动时间、术后进食流质饮食时间、术后住院时间短于对照组(P<0.05)。结论在治疗进展期胃癌患者时,采取腹腔镜全胃切除胃癌根治术具有比较理想的治疗效果,且治疗安全性较高,患者手术后的恢复速度较快,值得推广。

Objective To analyze the effect of laparoscopic total gastrectomy on prognosis and postoperative complication rate of patients with advanced gastric cancer.Methods 90 patients with advanced gastric cancer who were treated in Lianshui People’s Hospital from January 2018 to February 2021 were selected as the research objects.Divided into the control group(n=45)and the observation group(n=45)based on the treatment mode of the patients.The control group adopted conventional open surgery,and the observation group adopted laparoscopic total gastrectomy with radical gastrectomy.The patient’s treatment effect,postoperative complications,and perioperative indicators were recorded and compared between the two groups.Results The total clinical effective rate of the observation group was higher than that of the control group(P<0.05),and the complication rate was lower than that of the control group(P<0.05).The operation time of the observation group was longer than that of the control group(P<0.05),surgical incision length,intraoperative blood loss,number of injections of analgesics,postoperative anal exhaust time,first time to get out of bed,postoperative liquid diet time,postoperative time Hospital stay was shorter than the control group(P<0.05).Conclusion In the treatment of advanced gastric cancer patients,laparoscopic total gastrectomy for radical gastrectomy has a relatively ideal therapeutic effect,high safety and fast postoperative recovery,which is worthy of promotion.
