血清降钙素原在煤工尘肺合并下呼吸道感染患者中的临床效果及作用研究

Clinical Effect and Effect of Serum Procalcitonin in Patients with Coal Worker’s Pneumoconiosis Complicated with Lower Respiratory Tract infection

目的研究血清降钙素原(PCT)在煤工尘肺合并下呼吸道感染患者中的临床效果及作用。方法选取2019年7月—2020年7月在淮北市职业病防治院住院治疗的100例煤工尘肺合并下呼吸道感染患者为研究对象,以随机抽签方式分为对照组50例、试验组50例,对照组患者进行血常规、C反应蛋白、病原学以及影像学检测,试验组在对照组检测基础上加入血清PCT检验,比较两组诊断准确率、各检测结果、临床指标以及检验满意率。结果试验组检验准确率〔94.0%(47/50)〕高于对照组〔76.0%(38/50)〕(χ^(2)=6.353,P=0.012)。试验组白细胞计数、中性粒细胞计数、C反应蛋白、纤维蛋白原高于对照组(P<0.05)。在治疗后3 d、5 d、7 d G^(-)菌感染PCT均高于G^(+)菌感染(P<0.05)。试验组检验满意率〔94.0%(47/50)〕高于对照组〔80.0%(40/50)〕(χ^(2)=4.332,P=0.037)。结论煤工尘肺合并下呼吸道感染在常规检验的基础上加入血清PCT检测可以提升检验准确率,更好地对疾病进行判断,并对疾病病程进行观察,为疾病的治疗提供更加理想的条件,保证治疗的整体有效率,值得临床推广普及。

Objective To study the clinical effect and effect of serum procalcitonin in patients with coal worker’s pneumoconiosis complicated with lower respiratory tract infection.Methods 100 patients with coal worker’s pneumoconiosis complicated with lower respiratory tract infection hospitalized in Huaibei Occupational Disease Prevention Hospital from July 2019 to July 2020 were selected as the research object.They were randomly divided into 50 cases in the control group and 50 cases in the test group.The patients in the control group were tested for blood routine,C-reactive protein,etiology and imaging.The test group was added with serum PCT test on the basis of the test of the control group,The diagnostic accuracy,test results,clinical indexes and test satisfaction rate were compared between the two groups.Results The test accuracy of the test group〔94.0%(47/50)〕was higher than that of the control group〔76.0%(38/50)〕(χ^(2)=6.353,P=0.012).White blood cells counting,neutrophils counting,C-reactive protein and fibrinogen in the experimental group were higher than those in the control group(P<0.05).The PCT of G^(-) bacteria infection was higher than that of G^(+)bacteria infection at 3,5 and 7 days after treatment(P<0.05).The test satisfaction rate of the test group〔94.0%(47/50)〕was higher than that of the control group〔80.0%(40/50)〕(χ^(2)=4.332,P=0.037).Conclusion Coal workers pneumoconiosis with lower respiratory infection on the basis of routine inspection to join serum calcitonin original detection can improve test accuracy,better to judgment of disease,and carries on the observation of disease progression,provide more ideal conditions for the treatment of diseases,ensure the overall efficient of treatment,and worthy of clinical popularization.
