超声引导下腹横肌平面阻滞在小儿腹腔镜下腹股沟疝手术中的应用观察

Application of Ultrasound-Guided Horizontal Block in Laparoscopic Inguinal Hernia Surgery in Children

目的分析超声引导下腹横肌平面阻滞在小儿腹腔镜下腹股沟疝手术中的临床应用效果。方法选择2017年9月—2019年9月期间,在本院接受腹股沟疝手术治疗的患儿100例,随机分配为对照组与试验组,在所有患儿接受静脉注射全身麻醉的基础上,对照组患儿采用生理盐水注射,并在术中采用七氟醚持续麻醉,试验组患儿超声引导下双侧腹横肌平面阻滞,两侧各注射与对照组患儿相同剂量的罗哌卡因药物,比较两组患儿手术时间、苏醒时间以及疼痛评分,并比较两组患儿术后不同时间FLACC评分。结果两组患儿在手术治疗中采用不同麻醉药物对术后苏醒期情况造成较大差异,其中对照组患儿在手术时间、苏醒时间与试验组相比较差异无统计学意义(P>0.05),而患儿FLACC评分以及疼痛评分对比,试验组患儿显著优于对照组患儿,两组患儿比较差异有统计学意义(P<0.05)。结论临床采用超声引导下腹横肌平面阻滞在小儿腹腔镜下腹股沟疝治疗中效果良好,能够有效改善患儿术后恢复情况,对于保障患儿生命健康,提升手术治疗效果具有重要意义。

Objective To analyze the clinical effect of ultrasound-guided horizontal abdominal muscle block in laparoscopic inguinal hernia surgery in children.Methods From September 2017 to September 2019,100 children who received inguinal hernia surgery in the hospital were randomly assigned to the control group and the experimental group.On the basis of intravenous general anesthesia,the children in the control group were injected with normal saline,and sevoflurane was used for continuous anesthesia during the operation.The children in the experimental group received ultrasound-guided bilateral horizontal abdominal muscle block,The patients in the two groups were injected with the same dose of ropivacaine on both sides.The operation time,recovery time and pain score of the two groups were compared,and the flacc scores of the two groups at different times after operation were compared.Results There was a significant difference between the two groups in the recovery period after operation with different anesthetics.There was no statistical significance between the control group and the experimental group in the operation time and recovery time(P>0.05),while the experimental group was significantly better than the control group in flacc score and pain score,and the difference between the two groups was statistically significant Meaning(P<0.05).Conclusion The clinical use of ultrasound-guided horizontal abdominal muscle block in laparoscopic inguinal hernia treatment in children has a good effect,can effectively improve the recovery of children after surgery,for the protection of children’s life and health,improve the effect of surgery is of great significance.
