分层级延续性护理模式运用于糖尿病足患者中的效果及对足部并发症发生率的影响

Effect of Hierarchical Continuity of Care Model in Diabetic Foot Patients and Its Influence on the Incidence of Foot Complications

目的探讨分层级延续性护理模式运用于糖尿病足患者的效果及对足部并发症发生率的影响。方法选取2019年1月—2020年1月嘉兴市第一医院收治的80例糖尿病足患者,依据护理方法不同分为对照组与研究组,对照组进行常规护理,研究组进行分层级延续性护理。比较两组患者知识掌握率、并发症发生率、自我管理能力、生活质量、血糖水平。结果与对照组比较,研究组知识掌握率更高,并发症发生率更低(P<0.05);治疗前,两组自我管理能力评分间差异无统计学意义(P>0.05),治疗后,两组自我管理能力评分均明显升高,与对照组比较,研究组自我管理能力评分更高(P<0.05);与对照组比较,研究组生活质量评分更高(P<0.05);治疗前,两组血糖水平无明显差异(P>0.05),治疗后,两组血糖水平均明显降低,与对照组比较,研究组糖化血红蛋白、空腹血糖与餐后2h血糖指标更低(P<0.05);与对照组比较,研究组住院与溃疡愈合时间更短(P<0.05)。结论对糖尿病足患者进行分层级延续性护理,效果理想,可改善患者血糖水平,提高知识掌握率与生活质量,降低并发症发生率,可实现疾病自我管理,可缩短疾病恢复时间。因此,分层级延续性护理模式应在糖尿病足患者临床护理中广泛应用。

Objective To explore the effect of hierarchical continuous nursing model applied in diabetic foot patients and its influence on the incidence of foot complications.Methods 80 patients with diabetic foot admitted to our hospital from January 2019 to January 2020 were selected and divided into control group and research group according to different nursing methods.The control group received routine nursing and the research group received stratified continuous nursing.The knowledge mastery rate,incidence of complications,self-management ability,quality of life and blood glucose level of patients in two groups were compared.Results Compared with the control group,the study group had higher mastery rate and lower complications(P<0.05).Before treatment,there was no significant difference in the score of self-management ability between the two groups(P>0.05).After treatment,the score of self-management ability of the two groups was significantly increased,but compared with the control group,the score of self-management ability of the research group was higher(P<0.05).Compared with the control group,the quality of life score of the study group was higher(P<0.05).Before treatment,there was no significant difference in blood glucose levels between the two groups(P>0.05),and after treatment,blood glucose levels in both groups were significantly decreased,but compared with the control group,HbA1c,fasting blood glucose and 2 h postprandial blood glucose indexes in the study group were lower(P<0.05).Compared with the control group,the time of hospitalization and ulcer healing in the study group was shorter(P<0.05).Conclusion Hierarchical continuous nursing for diabetic foot patients has ideal effect,which can improve patients’blood glucose level,improve knowledge mastery rate and quality of life,reduce the incidence of complications,realize disease self-management,and shorten the recovery time of disease.Therefore,stratified continuous nursing mode should be widely used in clinical nursing of diabetic foot patients.
