门诊护理干预对提高老年糖尿病患者生活质量的效果研究

Study on the Effect of Outpatient Nursing Intervention on Improving the Quality of Life of Elderly Patients with Diabetes Mellitus

目的研究门诊护理干预对提高老年糖尿病患者生活质量的效果,为临床实践提供理论依据。方法选取2019年9月—2020年9月南部战区总医院收治的90例老年糖尿病患者为研究对象,随机分为对照组与研究组,各45例,对照组患者进行常规护理措施,研究组患者应用门诊护理干预。记录两组患者护理前后护理依从性、心理状态〔汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)〕、血糖水平变化情况(空腹血糖、餐后2 h血糖)、生活质量评分〔生活质量量表(SF-36量表)〕以及对护理工作(服务态度、业务技术、沟通交流、护患关系)满意度,并比较两组间的差异。结果护理前,两组患者护理依从性评分、SF-36评分、HAMA评分、HAMD评分、空腹血糖、餐后2 h血糖比较,差异均无统计学意义(P>0.05)。护理后,研究组护理依从性评分,SF-36评分,对服务态度、业务技术、沟通交流、护患关系满意度评分高于对照组,HAMA评分、HAMD评分、空腹血糖、餐后2 h血糖低于对照组(P<0.05)。结论老年糖尿病患者护理过程中应用门诊护理干预,能有效提高患者的护理依从性,消除负性情绪,改善生活质量,同时能够有效控制血糖,得到患者的满意评价,值得临床推广普及。

Objective To study the effect of outpatient nursing intervention on improving the quality of life of elderly patients with diabetes,and to provide theoretical basis for clinical practice.Methods Select 90 elderly diabetic patients admitted to the Southern Theater General Hospital from September 2019 to September 2020 as the research objects,and randomly divide them into the control group and the study group,with 45 cases in each group.The patients in the control group were given routine nursing measures,and the patients in the study group were given outpatient nursing intervention.Record the nursing compliance,psychological status(Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD)),changes in blood glucose levels(fasting blood glucose,blood glucose 2 h after a meal),quality of life scores〔life Quality scale(SF-36 scale)〕and satisfaction with nursing work(service attitude,business skills,communication,nurse-patient relationship),and compare the differences between the two groups.Results Before nursing,there was no significant difference in nursing compliance scores,SF-36 scores,HAMA scores,HAMD scores,fasting blood glucose,and blood glucose 2 h after meals between the two groups of patients(P>0.05).After nursing,the study group’s nursing compliance score,SF-36 score,service attitude,business skills,communication,and nurse-patient relationship satisfaction scores were higher than those of the control group,HAMA score,HAMD score,fasting blood glucose,and 2 h postprandial blood glucose Lower than the control group(P<0.05).Conclusion The application of outpatient nursing intervention in the nursing process of elderly patients with diabetes can effectively improve the nursing compliance of patients,eliminate negative emotions,improve the quality of life,effectively control blood sugar,and get the satisfaction evaluation of patients.The nursing effect is significant,which is worthy of clinical popularization.
