助产士一体式全程护理对初产妇自然分娩疼痛的影响观察

Observation on the Effect of Midwife Integrated Whole Course Nursing on Primipara's Natural Childbirth Pain

目的观察助产士一体式全程护理对初产妇自然分娩及分娩过程中疼痛情况的影响。方法选取2019年1月—2020年1月期间拟在医院进行自然分娩的初产妇128例,采用随机数字表法分为两组。对照组64例采用入院常规护理措施。观察组64例采用助产士一体式全程护理。采用疼痛数字评分法(NRS)评分评价产妇分娩期间疼痛程度,计算疼痛控制优良率,观察并记录各产程时间、分娩结局和产后并发症发生情况。结果观察组分娩时NRS评分低于对照组,疼痛控制优良率优于对照组(P<0.05)。观察组第一、二、三和总产程时间均短于对照组(P<0.05)。观察组自然分娩率高于对照组,会阴侧切助产率和中途转剖宫产率均低于对照组(P<0.05);观察组产后大出血、新生儿窒息发生率低于对照组(P<0.05);比较两组产后尿潴留发生率,差异无统计学意义(P>0.05)。结论对需进行自然分娩的初产妇实施助产士一体式全程护理,有利于缓解分娩疼痛,提高自然分娩率,并能减少术后并发症的发生。

Objective To observe the effect of midwife holistic nursing on primipara's natural delivery and pain during delivery.Methods A total of 128 primiparas scheduled for natural delivery in the hospital from January 2019 to January 2020 were selected and randomly divided into two groups.The control group was treated with routine nursing measures.64 cases in the observation group were treated with midwife integrated whole course nursing.The pain degree of parturient during delivery was evaluated by NRs score,and the excellent and good rate of pain control was calculated.The time of labor,delivery outcome and postpartum complications were observed and recorded.Results The NRS score of the observation group was lower than that of the control group,and the excellent and good rate of pain control was better than that of the control group(P<0.05).The first,second,third and total labor time of the observation group were shorter than those of the control group(P<0.05).The spontaneous delivery rate in the observation group was higher than that in the control group,and the rate of assisted lateral episiotomy and the rate of transvaginal cesarean section were lower than those in the control group(P<0.05);the incidence of postpartum hemorrhage and neonatal asphyxia in the observation group was lower than that in the control group(P<0.05);there was no significant difference in the incidence of postpartum urinary retention between the two groups(P>0.05).Conclusion The implementation of midwife integrated whole course nursing for primiparas who need natural childbirth is conducive to relieve labor pain,improve the natural delivery rate,and reduce the incidence of postoperative complications.
