经阴道瘢痕妊娠切除与经腹瘢痕妊娠切除的疗效评估

Evaluation of the Efficacy of Transvaginal Scar Pregnancy Resection and Transabdominal Scar Pregnancy Resection

目的探讨剖宫产术后子宫瘢痕妊娠采用经阴道病灶切除术与经腹病灶切除术治疗效果对比情况。方法选取剖宫产术后子宫瘢痕妊娠患者50例,均为贵州省毕节市第一人民医院2019年3月—2020年9月收治,采用数字表抽取法随机分组,就应用经阴道病灶切除术治疗(经阴道组,n=25)与应用经腹病灶切除治疗(经腹组,n=25)效果展开对比。结果经阴道组总有效率高于经腹组;术中出血量、手术操作用时间、住院天数、病灶缩小时间、β-hCG恢复正常时间、月经复潮时间均少于经腹组;生活质量各维度评分均高于经腹组;并发症率低于经腹组,差异均有统计学意义(P<0.05)。结论剖宫产术后子宫瘢痕妊娠采用经阴道病灶切除术治疗,可增强手术效果,减少术中出血量,缩短手术操作时间及术后恢复进程,并可提高患者生活质量,保障临床安全性。

Objective To investigate the comparison of the therapeutic effects of transvaginal lesion resection and transabdominal lesion resection for uterine scar pregnancy after cesarean section.Methods A total of 50 pregnancies with uterine scars after cesarean section were selected,all of whom were admitted to our hospital from April 2019 to April 2020.They were randomly grouped by digital table extraction,and treated with transvaginal lesion resection(transvaginal group,n=25)and the application of transabdominal lesion resection(transabdominal group,n=25).Results The total effective rate of the transvaginal group was higher than that of the transabdominal group;intraoperative blood loss,operation time,hospitalization days,lesion reduction time,β-hCG return to normal time,and menstrual regain time were all less than those of the transabdominal group.The scores of all dimensions of the quality of life were higher than those of the transabdominal group;the complication rate was lower than that of the transabdominal group,and the differences were statistically significant(P<0.05).Conclusion Uterine scar pregnancy after cesarean section is treated by transvaginal lesion resection,which can enhance the surgical effect,reduce intraoperative blood loss,shorten the operation time and postoperative recovery process,and improve the quality of life of patients and ensure clinical safety.
