摘要 目的通过分析急性白血病(AL)患者骨髓液表皮生长因子受体通路底物8(EPS8)、硫酸乙酰肝素酶(HPSE)以及基质金属蛋白酶(MMP)表达水平,评估3种指标联合检测对患者病情及预后的预测价值。方法将2017年10月—2018年10月于内蒙古科技大学包头医学院第一附属医院血液科治疗的60例患者分为两组:急性白血病患者为实验组(AL组)和缺铁性贫血患者为对照组,每组30例。收集每组患者骨髓标本,应用RT-PCR法检测EPS8、HPSE以及MMP-2的mRNA表达水平,应用流式细胞检测技术检测骨髓细胞表面EPS8、HPSE以及MMP-2的表达水平。通过比较两组EPS8、HPSE以及MMP-2的表达水平,应用多因素Logistic回归分析进一步探讨急性白血病完全缓解与未缓解组的因子表达及其临床相关意义。结果急性白血病患者骨髓液中EPS8、HPSE以及MMP-2因子表达水平与对照组存在显著差异(均P<0.05);多因素Logistic回归分析显示,MMP-2、EPS8是随访1年患者是否完全缓解的影响因素(P<0.05)。结论急性白血病患者骨髓EPS8、HPSE以及MMP-2表达水平升高,且联合检测患者骨髓白血病细胞中EPS8、MMP水平能较好地评估AL患者的临床转归及预后。 Objective To analysis expression of epidermal growth factor receptor pathway substrate 8(EPS8),heparan sulfate enzyme(HPSE)and matrix metalloproteinase(MMP)in patients with acute leukemia(AL),evaluate the predictive value of all three factors for the patient’s prognosis.Methods Sixty patients treated in the First Affiliated Hospital of Baotou Medical College,Inner Mongolia University of science and technology from 2017.10 to 2018.10..The patients were divided into two groups:acute leukemia group(experimental group,AL group)and iron deficiency anemia group(control group,N group).The number of cases in each group was 30.The bone marrow samples of each group were collected.The mRNA expression levels of EPS8,HPSE and MMP were detected by RT-PCR.The expression levels of EPS8,HPSE and MMP on bone marrow cells were detected by flow cytometry.After one year of followed-up,patients in the AL group were divided into the complete relieved(CR)group and the uncompleted group according to the outcome.The expression levels of EPS8,HPSE and MMP were compared between the AML and ALL with the control group respectively.Multivariate logistic regression was used to analyze the difference between EPS8,HPSE and MMP expression levels with patients who were in CR group and the uncompleted,analyze the clinical significance.Results There were significant differences for the expression levels of EPS8,HPSE and MMP between the two groups in bone marrow(P<0.05);Multivariate logistic regression analysis showed that EPS8 and MMP expression levels were risk factors for the prognosis of AL patient who were uncomplete relieved after one year of followed-up(P<0.05).Conclusion The expression levels of EPS8,HPSE and MMP in bone marrow of patients with acute leukemia are higher than the control group,and to detect both of EPS8 and MMP levels in patients can better evaluate the disease and prognosis of AL patients.
