上消化道出血行胃镜检查对患者病因诊断及治疗转归的积极影响

Positive Effect of Gastroscopy on Etiological Diagnosis and Treatment Outcome of Upper Gastrointestinal Hemorrhage

目的探究上消化道出血行胃镜检查对患者病因诊断及治疗转归的积极影响。方法选取2017年4月—2019年11月北京市朝阳区潘家园第二社区卫生服务中心接收的上消化道出血者114例为对象,对所有病例对象行急诊胃镜检查,通过对患者病因诊断情况的分析,对患者治疗疗效追踪分析,并阐明转归情况。结果在研究选取的114例患者中,经急诊胃镜诊断检查发现,急性胃炎率最高,其次分别是十二指肠溃疡、胃溃疡、食管胃底静脉曲张、其他、十二直肠癌(P<0.05)。对所有患者急诊胃镜检查发现,有60例伴有活动性出血情况,剩余54例患者出血情况已停止。急诊胃镜治疗止血成功率是99.12%,止血失败转院1例;7例十二指肠者急诊胃镜止血成功后转院实施治疗。结论对上消化道出血患者予以急诊胃镜检查,能有效诊出患者发病原因,且止血疗效佳,能及早治疗及发现上消化道出血,防止转院手术治疗情况发生,使外科手术率降低,患者预后改善。

Objective To explore the positive effect of gastroscopy on the etiological diagnosis and treatment outcome of upper gastrointestinal hemorrhage.Methods 114 patients with upper gastrointestinal hemorrhage received by the Second Community Health Service Center of Panjiayuan from April 2017 to November 2019 were selected as the subjects.All the patients underwent emergency gastroscopy.Through the analysis of the etiological diagnosis of the patients,the therapeutic effect of the patients was tracked and analyzed,and the outcome was clarified.Results Among the 114 patients selected in the study,emergency gastroscopy showed that the acute gastritis rate was the highest,followed by duodenal ulcer,gastric ulcer,esophageal and gastric varices,other,and duodenal cancer respectively(P<0.05).Emergency gastroscopy of all patients found that 60 patients were accompanied by active bleeding,and the remaining 54 patients had stopped bleeding.The success rate of emergency gastroscopy was 99.12%,and 1 case was transferred to hospital for hemostasis failure.Seven patients with duodenal diseases were transferred to hospital for treatment after successful emergency gastroscopy hemostasis.Conclusion Emergency gastroscopy for patients with upper gastrointestinal hemorrhage can effectively diagnose the cause of the disease,and has good hemostatic effect.It can treat and discover upper gastrointestinal hemorrhage as early as possible,prevent the occurrence of transfer to hospital for surgical treatment,reduce the surgical rate and improve the prognosis of patients.
