Beers标准(2019)和中国老年人潜在不适当用药判断标准(2017版)评估社区老年共病患者的不适当用药分析

Application of Beers Criteria(2019)and Chinese PIM Criteria(2017 version)to Evaluate Inappropriate Medication for Elderly Comorbid Patients in Communities

目的通过分析某社区家庭病床老年共病患者的用药,发现其潜在不适当用药,为推进老年患者合理用药提供参考。方法通过调阅2019-01-01至2019-12-31家庭病床老年共病患者的病历资料,筛选出年龄≥60岁且符合研究标准的患者47例。分别应用Beers标准(2019)和中国老年人潜在不适当用药判断标准(2017版),对45例老年患者的用药情况进行回顾性分析,分析潜在不适当用药情况。结果应用Beers标准(2019),涉及42例次潜在不适当用药;应用中国老年人潜在不适当用药判断标准(2017版),涉及22例次。结论老年共病患者出现潜在不适当用药比例较高,全科医师及临床药师在发现及降低潜在不适当用药、优化用药方案上应该发挥重要作用。

Objective To provide reference for propelling the proper medication on elder patients by analyze the medication of elderly comorbid patients in certain community home-based care and find the potential inappropriate mediction.Methods Through reviewing the medical records of elderly patients with comorbidity in family beds from January 1,2019 to December 31,2019,47 patients aged over 60 years and meeting the research standards were selected.The Beers criteria(2019)and the judgment criteria for the potential inappropriate medication of the elderly in China(2017 version)were applied respectively to retrospectively analyze the medication situation of 45 elderly patients and analyze the potential inappropriate medication.Results Applying the Beers criterion involved 42 cases of PIM,and applying the Chinese criteria the number is 22.Conclusion The elderly patients with comorbidities have a higher proportion of potentially inappropriate medications.General practitioners and clinical pharmacists should play an important role in discovering and reducing PIM and optimizing medications.
