社区骨质疏松症患者医护联合专病护理干预效果的研究

Study on the Effect of Community-based Nursing Interventions for Patients with Osteoporosis in Community

目的探讨社区医护联合专病护理干预对骨质疏松症患者提高治疗依从性,降低跌倒发生率。方法选取在枫林街道社区卫生服务中心门诊(在2017年4月-2018年4月)就诊的120例骨质疏松症老年患者作为研究对象,随机分为对照组和干预组,各60例。对照组:遵医嘱指导患者常规抗骨质疏松治,发放宣传资料、现场答疑及健康宣教、运动训练;干预组:在对照组的基础上,成立医护联合专病护理小组及俱乐部,给予个性化健康教育处方、小组授课讨论、电话随访、运动训练干预。比较干预后第3个月、第6个月、第9个月两组骨质疏松症老年患者治疗依从性,平衡力,起立-行走测试,跌倒功效量表。结果干预组骨质疏松症老年患者治疗依从性明显优于对照组,干预组的平衡力比对照组明显增长(P<0.05),起立-步行试验使用时间明显少于对照组(P<0.05)。跌倒功效量表均分比对照组增高(P<0.05)。结论医护联合专病护理干预给予骨质疏松症患者个性化,专业化的专病护理,提高患者治疗依从性,增强患者防跌倒意识,通过功能训练,提高平衡力,减低跌倒风险,是社区骨质疏松症患者防跌倒的有效措施。

Objective To explore community health care combined with special disease nursing intervention to improve treatment compliance and reduce the incidence of falls in patients with osteoporosis.Methods 120 elderly patients with osteoporosis who were treated in the outpatient clinic of Fenglin Sub-district Community Health Service(April 2017-April 2018)were randomly divided into control group and intervention group,60 cases in each group.Follow the doctor’s advice to guide patients to routine anti-osteoporosis treatment,distribute publicity materials,on-site Q&A and health education,and exercise training;Intervention group:On the basis of the control group,set up a medical combined disease care team and club to give personalized health education Prescription,group teaching discussion,telephone follow-up,exercise training intervention.The third month,the sixth month,and the ninth month after intervention,the elderly patients with osteoporosis were treated for compliance,balance,standing-walking test,and fall efficacy scale.Results The treatment compliance of elderly patients with osteoporosis in the intervention group was significantly better than that of the control group.The balance of the intervention group was significantly higher than that of the control group(P<0.05),and the time of the standing-walking test was significantly less than that of the control group(P<0.05).The fall effect scale was higher than that in the control group(P<0.05).Conclusion Medical combined with special disease nursing intervention to give personalized and specialized treatment for patients with osteoporosis,improve patient compliance,enhance patient’s anti-fall consciousness,improve balance by functional training,reduce risk of falls,is a community bone Effective measures to prevent falls in patients with loosening.
