腹膜外腹腔镜疝气修补术治疗老年腹股沟疝的临床有效性探究

Clinical Effectiveness of Extraperitoneal Laparoscopic Hernioplasty in the Treatment of Elderly Inguinal Hernia

目的探讨老年腹股沟疝采用腹膜外腹腔镜疝气修补术治疗的临床效果及安全性。方法于2017年1月—2019年1月期间在浙江省桐乡市第一人民医院治疗的老年腹股沟疝患者中选取80例,随机分为两组,各40例。研究组应用腹膜外腹腔镜疝气修补术治疗,对照组采用传统疝修补术治疗,比较两组治疗效果及并发症发生情况。结果研究组术中出血量显著少于对照组,研究组手术时间、下床活动时间、住院时间分别为(58.7±4.9)min、(27.3±3.6)h和(4.9±1.2)d,与对照组各指标比较均显著较短(P<0.05)。研究组术后第3天的VAS评分为(2.5±0.7)分,低于对照组的(3.9±1.2)分,差异有统计学意义(P<0.05)。研究组治疗有效率为95.0%,对照组有效率为72.5%,组间差异有统计学意义(P<0.05)。研究组与对照组并发症发生率分别为2.5%和20.0%,差异有统计学意义(P<0.05)。结论腹膜外腹腔镜疝气修补术应用于老年腹股沟疝患者,可有效缩短患者手术时间和下床活动时间,减轻患者术后疼痛,临床治疗效果显著,且并发症少,安全性高,值得推广。

Objective To investigate the clinical effect and safety of laparoscopic extraperitoneal hernioplasty for elderly inguinal hernia.Methods 80 elderly patients with inguinal hernia treated in First People’s Hospital of Tongxiang City from January 2017 to January 2019 were divided into two groups,40 cases in each group.The study group was treated with extraperitoneal laparoscopic herniorrhaphy,while the control group was treated with traditional herniorrhaphy.The therapeutic effect and complications of the two groups were compared.Results The amount of intraoperative bleeding in the study group was significantly less than that in the control group.The operation time,the time of getting out of bed and the time of hospitalization in the study group were(58.7±4.9)minutes,(27.3±3.6)hours and(4.9±1.2)days,respectively.The indexes in the study group were significantly shorter than those in the control group(P<0.05).The VAS score of the study group on the third day after operation was(2.5±0.7),lower than that of the control group(3.9±1.2),the difference was significant(P<0.05).The effective rate was 95.0%in the study group and 72.5%in the control group.There was a significant difference between the two groups(P<0.05).The incidence of complications was 2.5%in the study group and 20.0%in the control group(P<0.05).Conclusion Extraperitoneal laparoscopic herniorrhaphy can effectively shorten the operation time and ambulatory time of elderly patients with inguinal hernia,relieve the pain of patients after operation.The clinical effect is remarkable,with fewer complications and high safety.It is worth popularizing.
