铁剂静脉注射与铁剂口服治疗胃肠肿瘤相关性贫血疗效比较

Comparison of the efficacy of intravenous injection iron and oral iron treatment on gastrointestinal tumor-associated anemia

目的比较铁剂静脉注射与铁剂口服治疗胃肠肿瘤相关性贫血的效果。方法选取2017年3月-2018年5月在重庆市綦江区人民医院肿瘤科进行治疗的胃肠肿瘤相关性贫血患者200例,参照随机数字表法分为2组,观察组患者100例经铁剂静脉注射治疗,对照组患者100例经铁剂口服治疗,比较2组患者治疗前后血常规指标[血红蛋白(Hb)、血细胞比容(HCT)、血清铁蛋白(SF)、转铁蛋白饱和度(TSAT)]、生活质量评分及不良反应(发热、恶心、呕吐、便秘)发生情况。结果治疗2个月后,2组患者Hb、HCT、SF及TSAT水平均高于治疗前(P<0.05),且观察组高于对照组(P<0.01);观察组生活质量评分高于对照组(P<0.01);观察组患者不良反应总发生率低于对照组(P<0.05)。结论铁剂静脉注射治疗胃肠肿瘤相关性贫血的效果优于铁剂口服治疗,有利于提高患者的生活质量,改善患者预后状况,且具有较高的安全性。

Objective To compare the effect of intravenous injection iron and oral iron treatment on gastrointestinal tumor-associated anemia.Methods 200 cases with gastrointestinal tumor-associated anemia treated in the Department of Oncology,Qijiang District People’s Hospital,Chongqing from March 2017 to May 2018 were selected,and they were divided into 2 groups according to the random number table method,the observation group(100 cases)was treated with iron intravenous injection,and the control group(100 cases)was treated with iron oral,the blood routine indexes[hemoglobin(Hb),hematocrit(HCT),serum ferritin(SF),transferrin saturation(TSAT)],the score of life quality and adverse reactions(fever,nausea,vomiting,constipation)of the two groups were compared.Results After 2 months of treatment,the levels of Hb,HCT,SF and TSAT in the two groups were higher than before treatment(P<0.05),and the observation group was higher than the control group(P<0.01);The score of life quality of the observation group was higher than that of the control group(P<0.01);The total incidence of adverse reactions in the observation group was lower than that of the control group(P<0.05).Conclusion The effect of intravenous injection iron on gastrointestinal tumor-associated anemia is better than oral iron,it is beneficial to improve the quality of life of patients,improve the prognosis of patients,and it has high safety.
