甲状腺手术围术期应用抗菌药预防术后伤口感染的效果

Prophylactic effect of antibiotics on postoperative wound infection in thyroidectomy

目的探讨甲状腺手术围术期应用抗菌药预防术后伤口感染的效果。方法选取浮梁县人民医院2018年1月—2019年1月收治的接受甲状腺手术治疗的患者68例,按照随机数字表法分为对照组和观察组,各34例。两组均根据手术标准流程开展手术,对照组在围术期不应用抗菌药,观察组在围术期接受抗菌药治疗。比较两组术后切口愈合情况、住院时间、药品费用、并发症发生率;比较两组术前及术后1、2、3 d体温。结果两组术后切口甲级愈合率比较,差异无统计学意义(P>0.05)。观察组住院时间长于对照组,药品费用高于对照组(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。两组术前,术后1、2、3 d体温比较,差异无统计学意义(P>0.05)。结论甲状腺手术围术期应用抗菌药并不会明显减少术后伤口感染的发生,无法明显改善疾病预后,同时会延长住院时间,增加药品费用。

Objective To explore the effect of using antibiotics to postoperative wound infection during perioperative period of thyroid operation.Methods A total of 68 cases of patients who received thyroid surgery were selected from January 2018 to January 2019 in People’s Hospital of Fuliang County,which were divided into control group and observation group according to the random number table method,34 cases in each group.The two groups were operated according to the standard procedure of operation,the control group did not use antibiotics in the perioperative period,and the observation group received antibiotics treatment in the perioperative period.The wound healing,hospitalization time,drug cost and complication rate were compared between the two groups.The body temperature of the two groups before and 1,2,3 days after operation was compared.Results There was no significant difference in grade a healing rate of incision after operation between the two groups(P>0.05).The observation group of hospitalization time was longer than the control group,and the drug cost was higher than the control group(P<0.05).There was no significant difference in complication rate between the two groups(P>0.05).There was no significant difference in body temperature before and 1,2,3 days after operation between the two groups(P>0.05).Conclusion The application of antibiotics in the perioperative period of thyroid surgery will not significantly reduce the incidence of postoperative wound infection,it can not significantly improve the prognosis of the disease,at the same time,it will prolong the hospital stay and increase the cost of drugs.
