多层螺旋CT与DR摄影在隐匿性肋骨骨折诊断中的应用比较

Application comparison of MSCT and DR in the diagnosis of occult rib fracture

【目的】分析两种常用影像学技术在隐匿性肋骨骨折中的诊断价值。【方法】选取2018年1月至2020年4月收治的51例疑似肋骨骨折患者进行研究,患者均先行数字化X线摄影(DR)检查,后行多层螺旋CT(MSCT)检查,分析DR、MSCT两种检查方法阳性检出率、漏检率、检查时长及MSCT复检时间窗。【结果】51例患者共有91处隐匿性肋骨骨折发生;DR检出隐匿性肋骨骨折39例,72处,有12例19处漏检,阳性检出率为79.12%,漏检率为20.88%;MSCT检出隐匿性肋骨骨折48例,87处,有3例4处漏检,阳性检出率为95.60%,漏检率为4.40%;与DR相比,MSCT阳性检出率较高(P<0.05),漏诊率较低(P<0.05);与DR相比,MSCT检查用时较短(P<0.05);第3、4、5、6周MSCT复查与初诊结果相比,差异均有统计学意义(P<0.05)。【结论】MSCT与DR相比,隐匿性肋骨骨折阳性检出率更高,用时更短,有助于及时进行临床救治。

【Objective】To analyze the diagnostic value of two common imaging techniques in occult rib fracture.【Methods】From January 2018 to April 2020,a total of 51 cases of suspected rib fracture admitted in our hospital were studied.All patients underwent digital radiography(DR)examination,then multisliecs helieal CT(MSCT)was performed.The positive detection rate,missed detection rate and examination time of DR and MSCT were analyzed and compared,and the time window of MSCT reexamination was analyzed.【Results】A total of 91 parts of occult rib fracture occurred in 51 patients.DR detection showed that 39 cases had 72 parts of occult rib fracture,and 12 cases(19 parts)were missed in detection.The positive detection rate was 79.12%,and the missed detection rate was20.88%.MSCT detection showed that 48 cases had 87 parts of occult rib fracture,and 3 cases(4 parts)were missed in detection.The positive detection rate was 95.60%,and the missed detection rate was 4.40%.Compared with DR,positive detection rate of MSCT was higher(P<0.05)and missed diagnosis rate was lower(P<0.05),and the examination time of MSCT was shorter(P<0.05).There were statistically significant differences between MSCT reexamination and initial diagnosis results at week 3,4,5 and 6(P<0.05).【Conclusion】Compared with DR,MSCT has higher positive detection rate and shorter time for occult rib fracture,which is helpful for clinical treatment in time.
