生大黄保留灌肠联合生长抑素治疗急性胰腺炎效果观察

Study of raw rhubarb retention enema combined with somatostatin in the treatment of acute pancreatitis

【目的】探讨生大黄保留灌肠联合生长抑素治疗急性胰腺炎及并发症的疗效及对胃肠功能的影响。【方法】将2016年2月至2019年4月的100例急性胰腺炎患者,分为对照组和研究组,每组50例。对照组在常规治疗的基础上给予生长抑素治疗,研究组在对照组治疗的基础上给予生大黄保留灌肠治疗。比较两组患者的胃肠功能恢复情况、治疗前后的炎症因子水平、并发症发生情况以及临床疗效。【结果】与对照组比较,研究组患者治疗后的腹胀和腹痛缓解时间、肠鸣音恢复时间以及首次排气时间均较短,APACHEⅡ评分、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)、C反应蛋白(C-reactive protein,CRP)、白细胞介素-6(interleukin-6,IL-6)水平、并发症的总发生率较低,临床疗效较好,差异有统计学意义(P<0.05)。【结论】在常规治疗的基础上,给予急性胰腺炎患者生大黄保留灌肠联合生长抑素治疗,能够显著缓解机体的炎症反应水平、降低并发症的发生情况、促进胃肠功能的恢复,临床疗效较好,值得推广使用。

【Objective】To explore the curative effect of raw rhubarb retention enema combined with somatostatin on acute pancreatitis and its complications and its effect on gastrointestinal function..【Methods】A total of 100 patients with acute pancreatitis who were treated in our hospital from February 2016 to April 2019 were selected as the subjects in this study and divided into the control group and research group.The patients in the control group(n=50)were treated with somatostatin on the basis of conventional treatment,and the patients in the research group(n=50)were treated with rhubarb retention enema on the basis of the treatment of the control group.The recovery of gastrointestinal function,levels of inflammatory factors,occurrence of complications and clinical efficacy were compared between the two groups.【Results】After treatment,the abdominal distention and pain relief time,bowel sound recovery time and first exhaust time in the research group were significantly shorter than those in the control group,and the APACHEⅡscore,levels of TNF-α,CRP,IL-6 and total incidence of complications in the research group were significantly lower than those in the control group,and the clinical efficacy was significantly better than that in the control group(P<0.05).【Conclusion】On the basis of conventional treatment,raw rhubarb retention enema combined with somatostatin can significantly reduce the body’s inflammatory response level and occurrence of complications,promote the recovery of gastrointestinal function in patients with acute pancreatitis.Its clinical efficacy is good and worthy of promotion.
