养血清脑颗粒联合高压氧治疗血管性痴呆的临床研究

Clinical study of Yangxue Qingnao granules combined with hyperbaric oxygen in the treatment of vascular dementia

【目的】观察养血清脑颗粒联合高压氧治疗血管性痴呆(vascular dementia,VD)的临床效果。【方法】以本院2015年6月至2018年12月收治的128例VD患者作为研究对象,随机分成观察组和对照组,每组64例。对照组在常规治疗基础上给予养血清脑颗粒治疗,观察组在对照组的基础上加用高压氧治疗。治疗3个月后,比较两组患者的临床总疗效,简易智力状态检查量表(mini-mental state examimation,MMSE)、临床痴呆评定量表(clinical dementia rating,CDR)、日常生活自理能力量表(activity of daily living scale,ADL)评分,血液流变学及血流动力学指标。【结果】观察组治疗效果优于对照组(u=3.202,P=0.001)。治疗后,观察组MMSE评分、ADL评分、CDR评分变化明显优于对照组(P<0.05);观察组的血浆黏度、红细胞聚集指数、全血低切还原黏度及全血高切还原黏度指标较对照组显著降低(P<0.05),且大脑中动脉、大脑前动脉、大脑后动脉、基底动脉、椎动脉的平均血流速度均较对照组升高明显(P<0.05)。【结论】养血清脑颗粒联合高压氧治疗VD疗效显著,能明显改善患者的认知功能,值得临床推广应用。

【Objective】To observe the clinical effect of Yangxue Qingnao granules combined with hyperbaric oxygen in the treatment of vascular dementia(VD).【Methods】A total of 128 patients with VD in our hospital from June 2015 to December 2018 were selected as research objects.They were randomly divided into the control group and observation group,64 cases per group.On the basis of routine treatment,the patients in the control group were treated with Yangxue Qingnao granules,and those in the observation group were treated with hyperbaric oxygen based on the treatment of the control group.After 3 months of treatment,the total clinical efficacy between the two groups was compared,and the mini-mental state examination table(MMSE)and Clinical Dementia Rating(CDR)Scale,activity of daily living scale(ADL)score,blood rheology and hemodynamics indexes were compared between the two groups.【Results】The therapeutic efficacy in the observation group was better than that in the control group(u=3.202,P=0.001).After treatment,the changes of MMSE score,ADL score and CDR score in the observation group were significantly better than those in the control group(P<0.05).The plasma viscosity,erythrocyte aggregation index,whole blood low-cut reduction viscosity and whole blood high-cut reduction viscosity in the observation group were significantly lower than those in the control group(P<0.05),and the mean blood flow velocity of middle cerebral artery,anterior cerebral artery,posterior cerebral artery,basilar artery and vertebral artery was significantly higher than that in the control group(P<0.05).【Conclusion】Yangxue Qingnao granules combined with hyperbaric oxygen is effective in the treatment of VD,and can significantly improve the cognitive function of patients.It is worthy of clinical application.
