某武警机关门诊部抗高血压药处方用药情况分析

Analysis of prescription medication of antihypertensive drugs in the outpatient clinic of an armed police organ

【目的】了解某武警机关门诊部抗高血压药的处方用药情况,为临床合理用药及药品管理提供依据,更好地满足官兵卫生保健需求。【方法】随机抽取门诊部2019年1月至2019年8月的高血压处方1200张,采用用药频度(defined daily doses,DDDs)、日均费用(defined daily cost,DDC)和药物利用指数(drug utilization index,DUI),对抗高血压药的使用及联合用药情况进行统计分析。【结果】经处方分析,抗高血压药的使用与年龄具有一定相关性;各类药品以处方频率和DDDs排序:钙通道阻滞剂(calcium antagonistscalcium channel blockers,CCB)>血管紧张素Ⅱ受体拮抗剂(angiotensin receptor blocker,ARB)>β受体拮抗剂(beta receptor antagonist,β-RB)>复方制剂>血管紧张素转换酶抑制剂(angiotesion converting enzyme inhibitors,ACEI);在17种单药品种中,消耗金额和DDDs最高的是苯磺酸氨氯地平片,DDC最高的是缬沙坦氨氯地平片,DUI≤1的药物占41%,DUI>1的药物占59%;处方以单一用药为主,联合用药中二联用药占比最高,最常见药物组合是CCB+ARB及CCB+β-RB。【结论】本门诊部抗高血压药的应用整体基本合理,但部分药物存在不合理使用情况,需要进一步加强监督管理。

【Objective】To analyze the prescription medication of antihypertensive drugs in the outpatient clinic of an armed police organ,and provide further evidence for clinical rational drug use and drug administration and meet the demands for health care of officers and soldiers.【Methods】A total of 1200 prescriptions of hypertension were randomly selected from January 2019 to August2019,and defined daily doses(DDDs),defined daily cost(DDC)and drug utilization index(DUI)were used to analyze the use of antihypertensive drugs and their combination.【Results】By analyzing the prescriptions,the use of antihypertensive drugs was associated with age.Antihypertensive drugs were ranked by prescription frequency and DDDs:CCB>ARB>β-RB>SPC>ACEI.Among17 kinds of single drugs,amlodipine besylate tablets showed the largest consumption amount and the highest DDDs,and valsartan amlodipine tablets had the highest DDC.The constituent ratio of the drugs(DUI≤1)was 41%,while that of the drugs(DUI>1)was 59%.Single drug accounted for the most prescriptions,and the most common drug combination was the combination of two drugs,such as CCB+ARB and CCB+β-RB.【Conclusion】The application of antihypertensive drugs in clinic is basically reasonable,but some kinds of drugs are still used unreasonably.Therefore,it is necessary to strengthen the supervision and administration of drugs.
