经尿道前列腺绿激光汽化切除术与前列腺电切术治疗体积小于100mL良性前列腺增生的疗效对比分析

Comparative analysis of transurethral green laser vaporization of prostate and ransurethral resection of the prostate for benign prostatic hyperplasia with less than 100mL volume

目的:对比经尿道前列腺绿激光汽化切除术与前列腺电切术(TURP)治疗体积小于100 mL良性前列腺增生(BPH)的疗效及安全性。方法:回顾性分析2018年7月—2020年6月手术治疗80例BPH体积小于100 mL患者的临床资料,其中40例患者纳入对照组,采用TURP术,40例患者纳入观察组,采用经尿道绿激光汽化切除术,比较两组患者的手术时间、术中出血量、术后留置导尿管时间、膀胱冲洗时间、术后住院时间,最大尿流率(Q_(max) )、国际前列腺症状评分(IPSS)、生活质量评分(QOL)、术后并发症发生率。结果:两组患者手术时间、术后1个月Q_(max)、IPSS、QOL差异无统计学意义,但与对照组相比,观察组术中出血量明显减少,膀胱冲洗时间、留置尿管及术后住院时间更短。结论:绿激光前列腺汽化切除术与TURP术都可获得良好的治疗效果,但绿激光汽化切除术相比TURP术出血量更少,术后恢复更快,更加安全。

Objective:To compare the efficacy and safety of transurethral green laser vaporization resection of prostate and ransurethral resection of the prostate(TURP)for benign prostatic hyperplasia(BPH)with volume less than 100 mL.Methods:A retrospective analysis was performed on 80 BPH patients with volume less than 100 mL from July 2018 to June 2020.A total of 40 patients in the control group were treated by transurethral resection of prostate and 40 patients in the observation group were treated by transurethral laser vaporization.The operation time,intraoperative bleeding,postoperative indwelling catheter time,bladder irrigation time,postoperative hospitalization time,maximum urinary flow rate(Q;),international prostate symptom score(IPSS),quality of life score(QOL),incidence of postoperative complications were analyzed.Results:There was no significant difference in operation time and 1-month post-operative Q;,IPSS,QOL between the two groups.The blood loss during operation in the observation group was significantly reduced,and the time of bladder washing and indwelling catheter was significantly shortened as compared with those in the control group.Conclusion:Both green laser prostatectomy and TURP can achieve good therapeutic effects,but the amount of intraoperative bleeding is less and the postoperative recovery is faster by green laser prostatectomy,which is a safer surgical treatment than TURP.
