地龙散联合推拿牵引治疗血瘀型腰椎间盘突出症的临床观察

Clinical Observation of Dilong Powder Combined with Massage Traction in the Treatment of Blood stasis Lumbar Disc Herniation

目的:探讨地龙散联合推拿牵引治疗血瘀型腰椎间盘突出症的疗效。方法:选取2021年10月—2022年10月在湖南省直中医院脊柱外科住院的80例血瘀型腰椎间盘突出症患者为研究对象,随机分为治疗组、对照组,各40例。对照组予推拿牵引治疗,治疗组予地龙散内服+推拿牵引治疗。比较两组治疗前后的疼痛评分(VAS)、日本骨科协会评分(JOA)、白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)及用药安全性。结果:治疗组有效率92.50%(37/40),明显高于对照组的72.50%(29/40);两组治疗后的VAS及JOA评分均较前降低,治疗组明显优于对照组;且治疗组治疗后的血清IL-1β、TNF-α水平较对照组降低明显(P<0.05)。两组均无不良反应。结论:地龙散联合推拿牵引可明显加强血瘀型腰椎间盘突出症患者症状的缓解及腰椎功能改善,提高治疗效果,降低血清IL-1β及TNF-α水平,且安全性良好。

Objective:To explore the efficacy of Dilong powder combined with massage traction in the treatment of blood stasis lumbar disc herniation.Methods:80 patients with blood stasis lumbar disc herniation hospitalized in the Department of Spinal surgery of Hunan Provincial Hospital of traditional Chinese Medicine from October 2021 to October 2022 were randomly divided into treatment group(n=40)and control group(n=40).The control group was treated with massage traction,and the treatment group was treated with Dilong powder plus massage traction.The pain score(VAS),Japanese Orthopaedic Association score(JOA),Interleukin-1β(IL-1β),tumor Necrosis Factorα(TNF-α)and drug safety were compared between the two groups before and after treatment.Results:The effective rate of the treatment group was 92.5%(37/40),which was significantly higher than that of the control group 72.5%(29/40).The scores of VAS and JOA in the two groups after treatment were significantly lower than those in the control group,and the levels of serum IL-1βand TNF-αin the treatment group were significantly lower than those in the control group.There were no adverse reactions in both groups.Conclusion:Dilong Powder combined with massage traction can significantly enhance the relief of symptoms and the improvement of lumbar function in patients with blood stasis type of lumbar disc herniation,improve the therapeutic effect,reduce the level of serum IL-1 and TNF-α,and have good safety.
