中医药防治应激性溃疡的研究进展

Progress in the prevention and treatment of stress ulcer with traditional Chinese medicine

应激性溃疡(stressulcer,SU)一般指创伤、休克、手术后或者严重全身性感染时发生的急性胃炎,是一种急性胃黏膜病变,多伴有出血症状。在临床上运用西医治疗比中医治疗立竿见影,但是,消化性溃疡的复发率还是居高不下,遂有“一旦溃疡,终身溃疡”的说法。面对溃疡较高的复发率,如何预防复发,降低患病率,成为我们目前需要攻克的难题。西药在促进溃疡愈合方面效果更好更快,但不良反应较大,中医药具有提高溃疡愈合率、预防复发和并发症、安全有效、不良反应小等独特的优势。近些年来,大量中医学者在防治SU方面积累了较为丰富的经验,取得了不错的临床疗效。本文通过搜集近年来以中医药的方式干预应激后胃肠黏膜损伤资料,从中医方向对SU的病因病机、中医药治疗及中医特色治疗进行总结,为临床医生和科研工作者提供新的思路和研究视角,提高中医药治疗应激性溃疡的水平。

stressulcer(SU)generally refers to acute gastritis occurring after trauma,shock,surgery or serious systemic infection.It is an acute lesion of the gastric mucosa,often accompanied by bleeding symptoms.In clinical use of Western medicine treatment than the treatment effect of traditional Chinese medicine is more immediate,but the recurrence rate of peptic ulcer is still high,thus there is a"once ulcer,lifelong ulcer".Facing the high recurrence rate of ulcers,how to prevent SU recurrence and reduce the prevalence has become a difficult problem we need to overcome.Western medicine is better and faster in promoting ulcer healing,but western medicine has greater side effects.Traditional Chinese medicine has the unique advantages of improving ulcer healing,preventing recurrence and complications,being safe and effective with few side effects.In recent years,a large number of Chinese medicine scholars have accumulated rich experience in the prevention and treatment of SU,and achieved good clinical efficacy.In this paper,through the collection of relevant data on the intervention of gastrointestinal mucosal injury after stress by means of traditional Chinese medicine in recent years,the etiology,pathogenesis,TCM treatment and TCM characteristic treatment of SU were summarized from the direction of traditional Chinese medicine,providing new ideas and research perspectives for clinicians and researchers,and improving the level of TCM treatment of stress ulcer.
