吴茱萸热罨包在创伤骨科瘀斑处理中的应用研究

Application of Evodia rutae officinalis in the treatment of traumatic orthopedic ecchymosis

目的:探讨吴茱萸热罨包热敷在创伤骨科瘀斑处理中应用的效果。方法:选取2020年3月—2021年12月广州市花都区中西医结合医院骨科收治的骨创伤伴局部皮肤瘀斑患者248例,采用随机数表法分成对照组和研究组,每组124例。对照组给予常规护理,研究组加用吴茱萸热罨包热敷,观察两组疼痛症状、肿胀程度、瘀斑消退及不良反应。结果:两组VAS评分在组间、时点间、交互作用上差异均有统计学意义(P<0.001)。研究组疼痛缓解时间(2.72±0.80)d明显短于对照组的(3.47±0.94)d,差异有统计学意义(t=6.766,P<0.001)。对照组与研究组干预后肿胀程度均明显减轻(Z=9.182、11.332,P<0.001);组间比较显示,干预7 d后研究组肿胀程度轻于对照组,差异有统计学意义(Z=2.810,P=0.005)。干预后两组瘀斑评分均有明显改善(Z=11.892、13.831,P<0.001);干预后7 d组间比较,研究组瘀斑程度明显轻于对照组(Z=3.020,P=0.003)。研究组瘀斑完全消退率65.32%(81/124)明显高于对照组的50.00%(62/124)(χ^(2)=5.963,P=0.015)。研究组未出现不良反应。结论:吴茱萸热罨包热敷可快速解除创伤骨科瘀斑患者疼痛症状,减轻患处肿胀,促进瘀斑消退,安全性高、操作简便,值得临床推广。

Objective:To explore the effect of Evodia rutae officinalis hot compress in the treatment of traumatic orthopedic ecchymosis;Methods:Select patients with bone trauma and local skin ecchymosis who were admitted to the Department of Orthopedics,Huadu District Hospital of Integrated Traditional Chinese and Western Medicine,Guangzhou City from March 2020 to December 2021248 cases were divided into control group and study group by random number table method,124 cases in each group,the control group was given routine nursing,and the study group was additionally given hot compress with Evodia japonica.Results:The VAS scores of the two groups were significantly different between groups,time points,and interaction(P<0.001);the pain relief time in the study group(2.72±0.80)d was significantly shorter than that in the control group(3.47±0.94)d,the difference was statistically significant(t=6.766,P<0.001).The degree of swelling in the control group and the research group was significantly reduced after the intervention(Z=9.182,11.332,P<0.001);the comparison between the groups showed that the degree of swelling in the research group was lighter than that in the control group after 7 days of intervention,and the difference was statistically significant(Z=2.810,P=0.005);after the intervention,the ecchymosis scores of the two groups were significantly improved(Z=11.892,13.831,P<0.001);7 days after the intervention,the degree of ecchymosis in the study group was significantly lighter than that in the control group(Z=3.020),P=0.003;the complete regression rate of ecchymosis in the study group was 65.32%(81/124),which was significantly higher than 50%(62/124)(χ^(2)=5.963,P=0.015),and there were no adverse reactions in the study group.Conclusion:Evodia rutaecarpa hot compress can quickly relieve the pain symptoms of traumatic orthopedic patients with ecchymosis,reduce the swelling of the affected area,and promote the regression of ecchymosis.This method is safe and easy to operate,and is worthy of clinical promotion.
