益气活血汤配合西药低分子肝素对急性加重期肺心病患者心肺功能的影响

Influence of Yiqi huoxue decoction combined with Western medicine low molecular weight heparin on cardiopulmonary function in patients with acute exacerbation of pulmonary heart disease

目的:探讨益气活血汤联合低分子肝素对急性加重期肺心病患者心肺功能的影响。方法:纳入我院2017年12月~2020年1月收治的急性加重期肺心病患者96例,根据随机数表法,分成对照组、观察组,各48例。对照组给予低分子肝素治疗,观察组采用益气活血汤联合低分子肝素治疗,两组治疗2周为1个疗程。在治疗满1个疗程后,比较两组短期疗效,分别在治疗前、疗程结束后,经超声心动图分析两组心功能的变化,指标包括左室射血分数(LVEF)、心输出量(CO)、每搏输出量(SV)。经肺功能检测仪分析两组肺功能指标,包括第1秒用力呼气量(FEV1)、用力肺活量(FVC)、呼吸峰流速(PEF),记录两组不良反应情况。结果:观察组总有效率(93.75%,45/48)高于对照组(79.17%,38/48)(P<0.05)。两组治疗后LVEF、CO、SV高于治疗前,且观察组高于对照组(P<0.05)。两组治疗后FEV1、FVC、PEF高于治疗前,且观察组高于对照组(P<0.05)。观察组不良反应发生率为6.25%(3/48),与对照组的2.08%(1/48)比较差异无统计学意义(P>0.05)。结论:急性加重期肺心病采用益气活血汤联合低分子肝素治疗,能进一步改善心肺功能,从而提高总体疗效,安全性高。

Objective:To explore the influence of Yiqi huoxue decotioncombined with low molecular weight heparin on cardiopulmonary function in patients with acute exacerbation of pulmonary heart disease.Methods:A total of 96 patients with acute exacerbation of pul-monary heart disease admitted to our hospital from December 2017 to January 2020 were included and divided into the control group and the observation group according to the random number table method,with 48 cases in each group.The control group was treated with low molecular weight heparin,whereas the observation group was treated with Yiqi huoxue decoction combined with low molecular weight heparin.The two groups were treated for 2 weeks as a course of treatment.After 1 course of treatment,the short-term efficacy of the two groups was compared.Before treatment and after the end of course,the changes of cardiac function indicatorswere analyzed by echocardiography in the two groups,including left ventricular ejection fraction(LVEF),cardiac output(CO),and stroke volume(SV).Pulmonary function tester was used to analyze the pulmonary function indexes of the two groups,including forced expiratory volume in 1 s(FEV1),forced vital capacity(FVC)and peak expiratory flow(PEF).The adverse reactions of the two groups were recorded.Results:The total effective rate of the observation group was higher than that of the control group(93.75%vs.79.17%)(P<0.05).After treatment,the LVEF,CO,and SV of the two groups were higher than those before treatment,and the indicators in the observation group were higher than those in the control group(P<0.05).After treatment,the FEV1,FVC,and PEF of the two groups were higher than those before treatment,and the indexes in the observation group were higher than those in the control group(P<0.05).The adverse reaction rate in the observation group was not different from that in the control group(6.25%vs.2.08%)(P>0.05).Conclusion:For patients with acute exacerbation of pulmonary heart disease,Yiqi huoxue decoction combined with low molecular weight heparin can further improve car-diopulmonary function,and thereby enhance the overall efficacy,with high safety.
