产后48 h内穴位按摩疏通乳腺的重要性探究

Probe into the importance of point massage to dredge mammary gland within 48 hours postpartum

目的:探究产后48 h内行穴位按摩在乳腺疏通、促进乳汁分泌中的临床应用价值。方法:采取平行随机抽样法从我院产科2019年1月—2020年1月收治的产妇中选取128例作为研究对象,按护理方式不同分为观察组和对照组,各64例。产后为对照组实施常规护理干预,观察组产后48 h在常规护理基础上增加穴位按摩疏通乳腺,出院后随访30 d,比较两组产妇涨奶率、母乳喂养成功率、48 h血清泌乳素水平以及首次泌乳时间。结果:30 d随访结果示,观察组产妇涨奶率(28.13%,18/64)明显少于对照组(60.94%,39/64),差异有统计学意义(P<0.05);观察组母乳成功喂养率(90.63%,58/64)高于对照组(68.75%,44/64),差异有统计学意义(P<0.05);观察组产妇首次泌乳时间与血清泌乳素水平均优于对照组,组间差异有统计学意义(P<0.05)。结论:在产后48h内为产妇行穴位按摩疏通乳腺护理,即可促进泌乳素分泌,缩短首次泌乳时间,增加泌乳量,而且可有效改善产后涨奶情况,提高产妇生活质量,进而提高母乳喂养成功率。

Objective:To explore the clinical application value of acupoint massage within 48 hours postpartum in breast dredge and milk secretion.Methods:Adopt parallel random sampling method from our hospital obstetrics in January 2019 to January 2020 were selected from the maternal 128 cases as the research object,according to nursing methods of the different divided into observation group and control group,each an average of 64 cases,postpartum implement routine nursing intervention for the control group,observation group on the basis of conventional nursing increased 48 hours postpartum point massage dredge breast,courtyard after 30 days of follow-up,compared two groups of maternal milk up rate,success rate of breastfeeding,serum prolactin level 48 hours and lactation time for the first time.Results:The 30-day follow-up showed that the lactating rate of 28.13%(18/64)in the observation group was significantly lower than 60.94%(39/64)in the control group(P<0.05),and the difference was statistically significant.Compared with the control group,the rate of successful breastfeeding in the observation group was 90.63%(58/64)higher than 68.75%(44/64)(P<0.05).The first lactation time and serum prolactin level in the observation group were better than those in the control group,and the difference between the two groups was significant(P<0.05).Conclusion:The breast nursing can promote the secretion of prolactin,shorten the time of the first lactation and increase the amount of lactation,improve the postpartum lactation,improve the quality of life of the puerpera,and then improve the success rate of breastfeeding.
