激光前列腺剜除术与双极等离子前列腺剜除术治疗大体积BPH临床研究

Clinical Study of Transurethral 1470 nm Laser Prostatectomy and Bipolar Plasma Prostatectomy For Large Volume BPH

目的探讨经尿道1470nm激光前列腺剜除术与双极等离子前列腺剜除术(TUERP)治疗大体积良性前列腺增生(BPH)的临床价值。方法选取2017年2月至2019年11月就诊于我院的96例大体积BPH患者,按随机数字表法分为两组,各48例。试验组行经尿道1470 nm激光前列腺剜除术,参考组行PKERP。对比两组手术相关指标、国际前列腺症状评分(IPSS)、膀胱残余尿量(PVR)及并发症。结果试验组手术用时短于参考组,术中出血量、并发症总发生率低于参考组,差异有统计学意义(P<0.05);两组留置导尿管时间、术后住院时间、术后QOL、IPSS、PVR对比,差异无统计学意义(P>0.05)。结论经尿道1470 nm激光前列腺剜除术与TUERP均是治疗大体积BPH的有效方式,但前者创伤小、并发症少,手术安全性更高。

Objective To investigate the clinical value of transurethral 1470 nm laser prostate enucleation and urethra bipolar plasma prostate enucleation surgery(TUERP)in the treatment of large-volume benign prostatic hyperplasia(BPH).Methods A total of 96 large-scale BPH patients who were treated in our hospital from February 2017 to November 2019 were selected and divided into two groups according to the random number table method,with 48 cases each.The experimental group underwent transurethral 1470 nm laser prostate enucleation,and the reference group underwent TUERP.Surgical related indicators,International Prostate Symptom Score(IPSS),post-void residual(PVR),and complications were compared between the two groups.Results The operation time of the test group was shorter than that of the reference group,the intraoperative blood loss and the total incidence of complications were lower than those of the reference group,and the differences were statistically significant(P<0.05).There was no significant difference in the indwelling catheter time,hospital stay,postoperative QOL,IPSS,and PVR between the two groups(P>0.05).Conclusion Transurethral 1470 nm laser prostate enucleation and TUERP are effective methods for the treatment of large-volume BPH,but the former has less trauma,fewer complications,and higher surgical safety.
