多层螺旋CT征象对急性阑尾炎病理类型中的应用诊断分析

APPLICATION AND DIAGNOSIS OF MULTI-SLICE SPIRAL CT SIGNS IN PATHOLOGICAL TYPES OF ACUTE APPENDICITIS

目的:分析多层螺旋CT征象应用在急性阑尾炎病理类型诊断中的临床价值。方法:分析样本都是在我医院收入的急性阑尾炎患者中选择,共计200例,时间段控制2016-02~2018-02。所有患者执行多层螺旋CT平扫,对病理结果、CT征象、临床表现以及实验室检查结果等进行观察分析。结果:在200例患者中,8例为单纯性阑尾炎;177例为化脓性阑尾炎;15例为坏疽性阑尾炎。3种病例类型的阑尾炎患者CT征象情况,只有阑尾周围渗出表现出P<0.05;而阑尾粪石、回盲部淋巴结增大、盲肠壁增厚、阑尾管径均表现出P>0.05;患者白细胞计数对比呈现出P>0.05,而体温、中性粒细胞百分比以及阑尾管径进行对比呈现出P<0.05。结论:急性阑尾炎患者行相同的多层螺旋CT平扫后,针对不同病理分型的诊断,患者阑尾管径、中性粒细胞百分比、体温以及阑尾周围渗出情况拥有的指导价值非常高。

Objective:To analyze the clinical value of multi-slice spiral CT in the diagnosis of acute appendicitis.Methods:The analysis samples were all selected from the patients with acute appendicitis in our hospital,a total of 200 cases,and were controlled from February 2016 to February 2018.All patients underwent MSCT,and the pathological results,CT signs,clinical manifestations and laboratory examination results were observed and analyzed.Results:Among 200 patients,8 cases were simple appendicitis.177 cases were suppurative appendicitis.Fifteen cases were gangrenous appendicitis.CT findings of the patients with appendicitis in the three types of cases showed that only the exudation around the appendix showed a P value<0.05;The appendix bezoar,ileocecal lymph node enlargement,cecal wall thickening,and appendix tube diameter all showed P>0.05.Comparison of patients’white blood cell count showed a P>0.05,while comparison of body temperature,neutrophil percentage and appendix diameter showed a P 0.05.Conclusion:In patients with acute appendicitis,the same MSCT scan has a very high guiding value for the diagnosis of different pathological types,such as the diameter of the appendix,the percentage of neutrophils,body temperature and the exudation around the appendix.
