显微血管减压术中三叉神经根部浅表电凝治疗三叉神经痛的效果分析

Analysis of the effect of superficial electrocoagulation of trigeminal nerve root on trigeminal neuralgia during microvascular decompression

目的分析显微血管减压术中三叉神经根部浅表电凝治疗三叉神经痛的效果。方法选取164例三叉神经痛患者随机分为两组,对照组给予常规显微血管减压术治疗,观察组给予显微血管减压术且术中对三叉神经根部浅表电凝。比较两组手术指标、不同时刻疼痛评分、临床疗效、术后1个月内并发症发生率及随访6个月复发率。结果两组的手术时间、术中出血量相比差异均无统计学意义(P>0.05),观察组住院时间短于对照组(P<0.05);两组疼痛评分均随着时间的延长显著下降(P<0.05),术后观察组的疼痛评分均低于对照组(P<0.05);观察组总有效率高于对照组(P<0.05);两组术后1个月内听力下降、脑脊液漏、面瘫的发生率及复发率相比差异均无统计学意义(P>0.05),观察组面部麻木发生率高于对照组(P<0.05)。结论显微血管减压术中三叉神经根部浅表电凝治疗三叉神经痛可以缩短住院时间,减轻疼痛感,提高临床疗效,虽然可增加面部麻木发生率,但可快速恢复。

Objective To analyze on the effect of superficial electrocoagulation of trigeminal nerve root of trigeminal nerve root in microvascular decompression on trigeminal neuralgia.Methods A total of 164 patients with trigeminal neuralgia were selected and randomly divided into two groups.The control group received routine microvascular decompression and the observation group was treated with microvascular decompression and electrocoagulation(superficial electrocoagulation of trigeminal nerve root).The operative indexes,pain scores aft different moments,clinical efficacy,incidences of complications within 1 month after operation and recurrence rates within 6 months after following-up were compared between the two groups.Results There was no significant difference in operation time and bleeding volume between the two groups(P>0.05),and the length of stay in the observation group was shorter than that in the control group(P<0.05).Pain scores in both groups decreased significantly with the extension of time(P<0.05),which in the observation group were significantly lower than those in the control group(P<0.05).The total effective rate in observation group was significantly higher than the control group(P<0.05).There was no significant difference in the incidences of hearing loss,cerebrospinal fluid leakage and facial paralysis and recurrence rates between the two groups within one month after operation(P>0.05),and the incidence of facial numbness in the observation group was higher than that in the control group(P<0.05).Conclusion During microvascular decompression,superficial electrocoagulation of trigeminal nerve root can shorten the hospitalization time,reduce the pain and improve the clinical effect.Although it can increase the incidence of facial numbness,it can recover quickly.
