颅内单发海绵状血管瘤继发癫痫的伽玛刀治疗效果研究

The Effect of Gamma Knife treatment on intracranial Epileptogenic solitary cavernous angiomas

目的评估颅内单发海绵状血管瘤继发癫痫的伽玛刀治疗疗效。方法回顾性分析2015年1月至2017年4月期间在我院使用伽玛刀治疗的颅内单发海绵状血管瘤继发癫痫患者60例的临床资料,按Engel分级评价其癫痫改善情况,根据影像学检查评价病灶缩小情况及脑水肿发生情况。结果 60例患者获得完全随访,随访24~72个月,患者癫痫总改善率达80.00%(48/60),其中EngelI级24例(40%),EngelII级14例(23.3%),EngelIII级10例(38.8%),EngelIV级12例(20%),病灶体积缩小率43.3%(26/60),8.2%患者出现脑水肿(8/60)。结论伽玛刀可有效改善颅内单发海绵状血管瘤引起的癫痫,并发症少。

Objective To evaluate the effect of Gamma Knife treatment on intracranial solitary cavernous angioma with epilepsy.Methods The clinical data of 60 patients with intracranial solitary cavernous angioma with epilepsy treated by Gamma Knife radiosurgery in our hospital from January,2015 to April,2017 were retrospectively analyzed.All patients were evaluated for seizure improvement according to Engel grade.According to imaging examination to evaluate the lesions were reduced and cerebral edema occurred.Results All patients were followed up for 24 to 72 months.The total improvement rate of epilepsy was 80%(48/60),including 24 cases(40%) of Engel grade I,14 cases(23.3%) of Engel grade II,10 cases(38.8%) of Engel grade III,12 cases(20%) of Engel grade IV.Patients had a lesion reduction rate of 43.3%(26/60),and 8.2% had cerebral edema(8/60).Conclusion Gamma Knife treatment can effectively improve epilepsy caused by intracranial solitary cavernous angioma with few coplications.
