低能量超声波治疗男性勃起功能障碍疗效的荟萃分析

Efficacy of low energy ultrasound in the treatment of male erectile dysfunction:a meta-analysis

目的采用meta分析评估低能量超声波治疗男性勃起功能障碍(erectile dysfunction,ED)的临床治疗效果。方法通过计算机检索PubMed、Cochrane Library、Embase、中国知网、万方数据库、维普数据库。检索时间为建库日起至2022年7月1日。筛选有关低能量超声波治疗男性ED患者的随机对照双盲研究,并对文献进行质量评价与资料提取,采用ReviewManager 5.3对数据进行meta分析。结果纳入随机对照双盲研究(低能量超声波治疗vs.安慰治疗)11篇,与安慰治疗比较,采用低能量超声波治疗后,男性勃起功能可显著获益,国际勃起功能指数(international index of erectile function 5,IIEF5)在术后1[加权均数差(weighted mean difference,WMD)=4.01,95%CI 0.41~7.61,P=0.03]、6(WMD=1.92,95%CI 0.54~3.31,P<0.01)、12个月(WMD=2.22,95%CI 1.03~3.41,P<0.01)均有明显改善。有10项研究比较了患者采用低能量超声波治疗前后的IIEF5指标,结果显示患者术后IIEF5指标显著改善,差异有统计学意义(P<0.01)。经过低能量超声波治疗,勃起硬度评估(erection hardness score,EHS)>3分和获得满意性生活的患者数量明显多于安慰治疗组,差异有统计学意义(P<0.01)。结论低能量超声波治疗可帮助男性ED患者改善IIEF5评分,提高其性生活满意程度。

Objective To conduct a meta-analysis to evaluate the clinical therapeutic effects of low-intensity ultrasound therapy on male erectile dysfunction(ED).Methods A comprehensive search was performed across databases including PubMed,Cochrane Library,Embase,China National Knowledge Infrastructure,WanFang Database and VIP Database.The search period was from the date of establishment to July 1,2022.Randomized controlled double-blind studies related to low-intensity ultrasound therapy for male ED patients were selected for inclusion.Quality assessment and data extraction were conducted,and meta-analysis was performed using Review Manager 5.3.Results Eleven randomized controlled double-blind studies were included,comparing low-intensity ultrasound therapy with placebo treatment.Following low-intensity ultrasound therapy,significant improvements in male erectile function were observed,as evidenced by the international index of erectile function 5(IIEF5)scores at 1 month posttreatment[weighted mean difference(WMD)=4.01,95%CI 0.41-7.61,P=0.03],6 months(WMD=1.92,95%CI 0.54-3.31,P<0.01),and 12 months(WMD=2.22,95%CI 1.03-3.41,P<0.01).Ten studies were conducted to compare the IIEF5 scores of patients before and after low-energy ultrasound therapy.The results demonstrated a significant improvement in postoperative IIEF5 scores among patients,with a statistically significant difference(P<0.01).Following low-energy ultrasound treatment,the number of patients achieving an erection hardness score(EHS)>3 and satisfactory sexual activity was significantly higher than in the control group,with a statistically significant difference(P<0.01).Conclusion Low-intensity ultrasound therapy can assist male ED patients in improving IIEF5 scores and enhancing sexual satisfaction.
