“三师共管”模式对社区高血压性心脏病的管理效果

The effect of“three divisions co-management”model on the management of hypertensive heart disease in communities

目的探讨“三师共管”模式对社区高血压性心脏病的管理效果。方法选择2020年10月~2020年12月在潍坊市奎文区大虞社区确诊的高血压性心脏病患者为研究对象,随机分为实验组和对照组。实验组采用“三师共管”模式,对照组采用常规管理。每月随访1次,共12个月。评估两组患者1年后治疗依从性、血压控制率及心血管相关危险指标的变化。结果1年后,实验组治疗依从性和血压控制率显著高于对照组(P<0.01);吸烟率、BMI下降,运动人数增加,降压药联合使用率增加(P<0.05);总胆固醇、低密度脂蛋白降低(P<0.05);心电图和心脏超声结果好转(P<0.05)。结论“三师共管”模式能提高高血压性心脏病患者治疗依从性和血压控制率,降低心血管病发生危险,改善心脏功能。

Objective To explore the effect of“three divisions co-management”model on the management of hypertensive heart disease in communities.Methods From October 2020 to December 2020,patients with hypertensive heart disease diagnosed in the community clinic were selected as the study subjects and randomly divided into the experimental group and the control group.The experimental group adopted the“three divisions co-management”mode,while the control group adopted routine management.The follow-up was conducted once a month for a total of twelve months.The changes in treatment compliance,blood pressure control rate,and cardiovascular related risk indicators were evaluated in both groups after one year.Results After one year,the treatment compliance and blood pressure control rate in the experimental group were significantly higher than those in the control group(P<0.01);In the experimental group,the smoking rate and BMI decreased,the number of people exercising increased,and the combined use rate of antihypertensive drugs increased(P<0.05);The total cholesterol and low density lipoprotein significantly decreased(P<0.05);The results of electrocardiogram and cardiac ultrasound were significantly improved(P<0.05).Conclusion The model of“three divisions co-management”can improve the treatment compliance and blood pressure control rate of patients with hypertensive heart disease,reduce the risk of cardiovascular disease,and improve cardiac function.
