二维斑点追踪技术评价主动脉瓣置换术前、术后左室收缩功能 被引量:2

摘要 目的应用二维斑点追踪技术(2DSTI)评价主动脉瓣关闭不全(AI)患者瓣膜置换术(AVR)前、后左室整体及局部收缩功能变化。方法应用2DSTI技术对30例AI患者术前、术后左心室收缩功能进行分析,并与30例正常对照组进行比较。结果AVR术后,左室射血分数及左室结构与对照组之间差异无统计学意义(P>0.05);左室心肌纵向应变峰值较术前增加,但低于对照组,达峰时间较术前缩短,但较对照组延长,差异有统计学意义(P<0.05);左室各水平段纵向应变峰值及达峰时间均呈现为基底段>中间段、基底段>心尖段,差异有统计学意义(P<0.05)。结论2DSTI技术可以更准确地评价AI患者AVR术后心肌收缩力的变化及恢复情况。 Objective The two-dimensional speckle tracking imaging(2 DSTI)was used to evaluate the changes in the aortic valve replacement(AVR),the overall and local systolic function of the left ventricle.Methods Preoperative and postoperative left ventricular systolic function of 30 AI patients was analyzed by using2 DSTI technology,and compared with 30 normal controls.Results After AVR,no significant difference was found between left ventricular ejection fraction and left ventricular structure and control group(P>0.05).After surgery,the longitudinal strain of the left ventricular myocardial strain increased from the preoperative period,but it was still lower than that in the control group.The peak time was shorter than the preoperative period,but the difference was statistically significant(P<0.05).The longitudinal strain peak and peak time of the left ventricular horizontal segment showed a difference between the middle segment and the basal segment,the difference between the basal segment was greater than the cardiac segment(P<0.05).Conclusion 2 DSTIS can evaluate changes in and recovery of myocardial contractile force in patients with aortic valve closure.
