桂芍四妙汤联合复方沙棘籽油栓治疗霉菌性阴道炎疗效及对细胞因子水平影响

Efficacy of Guishao Simiao Decoction(桂芍四妙汤)Combined with Compound Seabuckthorn Seed Oil Suppository in the Treatment of Fungal Vaginitis and Its Influence on Cytokine Levels

目的探究与分析桂芍四妙汤联合复方沙棘籽油栓治疗霉菌性阴道炎的疗效及对细胞因子水平的影响。方法选取2019年5月—2020年5月收治的霉菌性阴道炎患者120例,采取随机数字表法分为对照组与观察组,每组各60例,对照组给予酮康唑胶囊联合外用制菌素阴道栓治疗,观察组给予桂芍四妙汤联合复方沙棘籽油栓治疗,两组均连续治疗1个月,对比两组临床疗效、不良反应、治疗前后外周血及阴道分泌物中细胞因子水平、停药后6个月内复发率。结果观察组与对照组相比临床总有效率较高,差异有统计学意义(P<0.05)。两组用药期间不良反应无明显差异(P>0.05)。两组治疗前相比外周血及阴道分泌物中IL-2、TNF-α、INF-γ、IL-4、IL-6、IL-10水平无明显差异(P>0.05)。观察组治疗后与对照组治疗后相比外周血及阴道分泌物中IL-2、TNF-α、INF-γ水平较高,IL-4、IL-6、IL-10水平较低,差异有统计学意义(P<0.05)。观察组与对照组相比随访6个月内复发率较低,差异有统计学意义(P<0.05)。结论桂芍四妙汤联合复方沙棘籽油栓治疗霉菌性阴道炎的临床疗效较为突出,未增加不良反应,同时可有效的调节外周血及阴道分泌物中细胞因子水平,停药后不容易复发、安全性较高、预后好。

Objective To explore and analyze the efficacy of Guishao Simiao Decoction(桂芍四妙汤)combined with compound seabuckthorn seed oil suppository in the treatment of fungal vaginitis and its influence on cytokine levels.Methods A total of 120 patients with fungal vaginitis admitted to our hospital from May 2019 to May 2020 were selected and divided into a control group and an observation group by a random number table,60 cases in each group,and the control group was given ketoconazole capsules combined with topical bacteriostatic vaginal suppositories,the observation group was treated with Guishao Simiao Decoction combined with compound seabuckthorn seed oil suppositories.Both groups were treated continuously for 1 month.The clinical efficacy,adverse reactions,peripheral blood and vaginal secretions before and after treatment were compared.The level of cytokine in the serum and the recurrence rate within 3 months after stopping the drug.Results Compared with the control group,the observation group had a higher clinical total effective rate,and the difference was statistically significant(P<0.05).There was no significant difference in adverse reactions between the two groups during medication(P>0.05).The levels of IL-2,TNF-α,INF-γ,IL-4,IL-6,and IL-10 in peripheral blood and vaginal secretions were not significantly different between the two groups before treatment(P>0.05).Compared with the control group after treatment,the observation group has higher levels of IL-2,TNF-α,and INF-γin peripheral blood and vaginal secretions,while the levels of IL-4,IL-6,and IL-10 are lower.There are differences statistically significant(P<0.05).The observation group had a lower recurrence rate within 3 months of follow-up compared with the control group,and the difference was statistically significant(P<0.05).Conclusion Guishao Simiao Decoction combined with compound seabuckthorn seed oil suppository has a prominent clinical effect in the treatment of fungal vaginitis without increasing adverse reactions.At the same time,it can effectively regulate the level of cytokines in peripheral blood and vaginal secretions.It is not easy after stopping the drug relapse,high safety,and good prognosis.
