针刺联合康复治疗在缺血性脑卒中患者肢体功能及日常生活能力恢复中应用效果研究

Application Effect of Acupuncture Combined with Rehabilitation Therapy in the Recovery of Limb Function and Activities of Daily Living in Patients with Ischemic Stroke

目的探讨针刺联合康复治疗在缺血性脑卒中患者肢体功能及日常生活能力恢复中的应用效果研究。方法将2015年1月—2020年11月收治的60例缺血性脑卒中患者随机分为两组,每组各30例,对照组单纯进行康复训练治疗,在此基础上,观察组进行针刺治疗,对比两组的临床疗效、简式Fugl-Meyer运动评分量表(FMA)、Berg平衡量表(BBS)、功能独立性评定量表(FIM)、改良Ashworth痉挛量表评分(MAS)、中风痉挛性偏瘫基于患者报告的结局(PRO)、Barthel指数(BI)、生活质量。结果观察组总有效率为93.33%,显著高于对照组的76.67%(P<0.05);观察组治疗后FMA评分、BBS评分、FIM评分明显高于对照组(P<0.05);观察组治疗后上肢MAS评分、下肢MAS评分明显低于对照组(P<0.05);观察组治疗后PRO评分明显低于对照组,BI指数明显高于对照组(P<0.05);观察组治疗后生理机能、社会功能、精神健康、总体健康等SF-36评分明显高于对照组(P<0.05)。结论针刺联合康复治疗在缺血性脑卒中患者肢体功能及日常生活能力恢复中的应用效果显著,能有效改善肢体整体功能,提高偏侧肌力,提升日常生活活动能力及生活质量,具有积极的现实临床意义。

Objective To explore the application effect of acupuncture combined with rehabilitation therapy in the recovery of limb function and activities of daily living in patients with ischemic stroke.Methods 60 patients with ischemic stroke treated in my hospital from January 2015 to November 2020 were randomly divided into two groups,30 cases in each group.The control group was treated with rehabilitation training only.On this basis,the observation group was treated with acupuncture.The clinical efficacy,FMA,BBS and functional independence,the scores of modified Ashworth spasticity scale(MAS),stroke spastic hemiplegia based on patient reported outcome(PRO),Barthel Index(BI),quality of life of the two groups were compared.Results The effective rate of the observation group was 93.33%,which was significantly higher than 76.67%of the control group(P<0.05);the FMA score,BBS score and FIM score of the observation group after treatment were significantly higher than those of the control group(P<0.05);the MAS score of the upper limb and lower limb of the observation group after treatment was significantly lower than that of the control group(P<0.05);the PRO score of the observation group was significantly lower than that of the control group,and the BI index was significantly higher than that of the control group;the SF-36 scores of the observation group were significantly higher than those of the control group(P<0.05).Conclusion Acupuncture combined with rehabilitation therapy has a significant effect in the recovery of limb function and activities of daily living in patients with ischemic stroke,which can effectively improve limb function,improve muscle strength,improve the ability of daily life and quality of life,which has positive clinical significance.
