基于关联规则、熵层次聚类法探索治疗小儿抽动症选穴规律

Exploration of the Law of Acupoint Selection Based on Association Rules and Entropy Hierarchical Clustering Method for Treating Tourette’s Syndrome in Children

目的基于关联规则、熵层次聚类法分析治疗小儿抽动症的选穴规律。方法对中国期刊全文数据库(CNKI)中应用针刺、灸法、穴位注射等方法治疗小儿抽动症的文献进行检索,应用中医传承辅助平台对所有处方的选穴规律进行分析。结果共纳入处方45首,涉及穴位90个,其中,用穴频次≥4次的穴位有27个,其中使用频次排名前5位的穴位分别为太冲(29次)、百会(28次)、风池(28次)、合谷(27次)、三阴交(19次);设置支持度=12,置信度=0.9,得到25对穴位组合,其中,排名前5位的高频穴位组合为合谷-太冲(25次)、百会-太冲(23次)、百会-风池(21次)、百会-合谷(21次)、风池-合谷(21次);得到3个穴位的核心组合24个,4个穴位的核心组合3个。结论针灸治疗小儿抽动症主要通过百会、太冲、合谷、三阴交等穴位起到疏肝解郁、补肾健脾的作用,并为临床发展新型处方提供参考。

Objective Based on association rules and entropy hierarchical clustering method to analyze the law of acupoint selection in the treatment of Tourette’s syndrome in children.Methods The Chinese Journal Full-text Database(CNKI)was used to retrieve literatures on acupuncture,moxibustion,acupoint injection and other methods for treating Tourette’s syndrome in children,and the traditional Chinese medicine assistant platform was used to analyze the rules of acupoint selection for all prescriptions.Results A total of 45 prescriptions were included,involving 90 acupoints,of which 27 were used with a frequency of≥4,of which the top five used frequencies were Taichong(LR3,29 times),Baihui(DU20,28 times),and Fengchi(GB20,28 times),Hegu(LI4,27 times),Sanyinjiao(SP6,19 times);set support=12,confidence=0.9,and get 25 pairs of acupoint combinations,of which the top five high-frequency acupoint combinations are Hegu(LI4)-Taichong(LR3)(25 times),Baihui(DU20)-Taichong(LR3)(23 times),Baihui(DU20)-Fengchi(GB20)(21 times),Baihui(DU20)-Hegu(LI4)(21 times),Fengchi(GB20)-Hegu(LI4)(21 times);24 core combinations of 3 acupoints and 4 acupoints 3 core combinations.Conclusion Acupuncture and moxibustion treatment of Tourette’s syndrome in children is mainly through Baihui(DU20),Taichong(LR3),Hegu(LI4),Sanyinjiao(SP6)and other acupoints to relieve liver and stagnation,nourish the kidney and spleen,and provide a reference for the development of new prescriptions for clinical development.
