电针俞募穴联合膀胱功能训练治疗脊髓损伤后尿潴留临床观察

Clinical Observation on Treatment of Urinary Retention after Spinal Cord Injury by Electroacupuncture at Shu and Mu Acuoints Combined with Bladder Function Training

目的观察电针俞募穴联合膀胱康复训练治疗脊髓损伤后尿潴留的疗效。方法选取2019年6月—2020年12月医院收治的脊髓损伤后尿潴留患者64例作为研究对象,将其随机分为治疗组(32例)和对照组(32例)。对照组予以基础治疗及膀胱功能训练,治疗组在对照组基础上结合电针俞募穴治疗。对比两组患者的排尿频次、残余尿量、单次排尿量、膀胱内压力和临床疗效以及尿路感染、国际下尿路症状评分(LUTS)等指标。结果每组各脱落2例,最终完成30例。与治疗前相比,两组治疗后的排尿频次、残余尿量、单次排尿量、膀胱内压力、尿路感染率及LUTS评分均有不同程度改善(P<0.05);与对照组相比,治疗组上述指标及临床疗效均优于对照组(P<0.05)。结论电针俞募穴联合膀胱功能训练治疗脊髓损伤后尿潴留疗效确切,可显著改善膀胱功能,降低患者尿路感染,且安全可靠,便于推广。

Objective To observe the curative effect of electroacupuncture at Shu and Mu acupoints combined with bladder rehabilitation training on urinary retention after spinal cord injury.Methods Sixty-four patients with urinary retention after spinal cord injury admitted to the hospital from June 2019 to December 2020 were selected as the research objects,and they were randomly divided into treatment group(32 cases)and control group(32 cases).The control group was given basic treatment and bladder function training,and the treatment group was combined with electroacupuncture at Shu and Mu acupoints on the basis of the control group.The urinary frequency,residual urine volume,single urinary volume,bladder pressure,clinical efficacy,urinary tract infection,international lower urinary tract symptom score(LUTS)and other indicators were compared between the two groups.Results Two cases fell off in each group,and 30 cases were finally completed.Compared with those before treatment,the frequency of urination,residual urine volume,single urination volume,bladder pressure,urinary tract infection rate and LUTS score of the two groups after treatment were improved to varying degrees(P<0.05).Compared with those of the control group,the above indicators and clinical efficacy of the treatment group were better than those of the control group(P<0.05).Conclusion Electroacupuncture at Shu and Mu acupoints combined with bladder function training has a definite effect on urinary retention after spinal cord injury.It can significantly improve bladder function and reduce urinary tract infections in patients.It is safe,reliable,and easy to promote.
