态靶辨证在2型糖尿病肠道湿热证中的运用——葛根芩连汤加云茯苓、生姜

Application of State-Target Strategy in Dampness-Heat Syndrome of Intestinal Tract of Type 2 Diabetes Mellitus——Gegen Qinlian Decoction(葛根芩连汤)with Fuling(Poria)and Shengjiang(Zingiberis Rhizoma Recens)

糖尿病是国际公认最为常见的慢性内科疾病之一,由其所引发的多种急慢性并发症时刻威胁着患病群体的身体健康和生存质量。对于糖尿病的治疗,现代医学主要采取口服、皮下注射药物降糖,效果明确但也不可避免地存在诸多局限。随着中医理论体系的不断完善和现代药理学研究成果的临床回归,中医药降糖之独特优势日益显现。仝小林院士提出“态靶因果”中医处方策略,对糖尿病进行重新分类、分期、分证,将其病程概括为郁、热、虚、损4个阶段,其中肠道湿热证是热阶段最为典型的证候,患者以大便黏稠、舌苔黄腻等为主要表现,治疗上在采用葛根芩连汤调整肠道湿热之偏态的同时,加用云茯苓利湿健脾,生姜辛开苦降,态靶结合,疗效甚佳。

Diabetes mellitus is recognized as one of the most common chronic medical diseases in the world.Many acute and chronic complications caused by diabetes mellitus threaten the health and quality of life of patients.For the treatment of diabetes,modern medicine mainly adopts oral and subcutaneous injection drugs to reduce glucose,which has clear effects but inevitably has many limitations.With the continuous improvement of the theoretical system of traditional Chinese medicine(TCM)and the clinical return of modern pharmacology research results,the unique advantages of TCM in hypoglycemia are increasingly emerging.Academician TONG Xiaolin proposed the TCM prescription strategy of“state-target cause and effect”,reclassified diabetes into four stages,namely depression,heat,deficiency,and loss.Among them,intestinal dampness-heat syndrome is the most typical syndrome in the heat stage,and the patients are mainly manifested by sticky stool and yellow and greasy tongue coating.In the treatment,Gegen Qinlian Decoction(葛根芩连汤)is used to adjust the deviation of intestinal dampness and heat,while Fuling(Poria)was used to invigorate spleen and eliminate dampness.Shengjiang(Zingiberis Rhizoma Recens)was used to open the orifices and make Qi descend,and the combination of state and target is effective.
