名医贾成文“头皮神八针”联合中药治疗顽固性失眠经验

JIA Chengwen′s Experience of “Scalp Spirit Eight Needling”Combined with Traditional Chinese Medicine in Treatment of Intractable Insomnia

顽固性失眠作为一种慢性重症睡眠障碍,其睡眠时间不足和质量低下的特点严重影响患者身心健康。现代医学治疗顽固性失眠主要以苯二氮卓受体激动剂、抗抑郁等口服药物治疗为主,然长期服用药物后所产生的不良反应、依赖性让患者痛苦不堪;因此顽固性失眠的治疗迫切需要一种安全有效的方法。总结贾成文教授以针刺结合口服中药治疗顽固性失眠的经验,为临床治疗此病提供一种安全性更高、不良反应少患者易于接受的新方法。贾教授总结出顽固性失眠病位在心,主要责之肝、胆两脏;认为“心、肝、之阳燥动而心神不安或肝失疏泄、肝气郁结,胆气失和,肝胆互扰”是发病病机,治疗以“平肝潜阳,养心安神”为法,具体以“头皮神八针”联合晚清医家费伯雄方甲乙归藏汤加减,内服中药外以针刺,身心兼调。贾教授基于肝胆论治疗顽固性失眠而获效验,为临床治疗顽固性失眠提供了宝贵的经验,值得推广。

As a chronic severe sleep disorder,intractable insomnia has the characteristics of insufficient sleep time and low quality,which seriously affects the physical and mental health of patients.Modern medicine mainly treats intractable insomnia with benzodiazepine receptor agonists,antidepressants and other oral drugs.However,the adverse reactions and dependence caused by taking drugs for a long time make patients miserable.Therefore,the treatment of intractable insomnia urgently needs a safe and effective method.This paper is to summarize Professor JIA Chengwen′s experience on treating intractable insomnia with acupuncture combined with oral traditional Chinese medicine and provide a new method with higher safety,less adverse reactions and easy acceptance for patients.Professor JIA has concluded that intractable insomnia lies in the heart and is mainly responsible for the liver and gallbladder.It is considered that the pathogenesis is“restlessness caused by the dryness of heart,liver and Yang,or the loss of drainage of liver,stagnation of liver Qi,disharmony of gallbladder Qi and mutual disturbance of liver and gallbladder”.The treatment is based on the method of“calming liver and latent Yang,nourishing heart and calming the mind”,specifically combined with“scalp spirit eight needling”and Jiayi Guizang Decoction(甲乙归藏汤)of FEI Boxiong,a doctor of the late Qing Dynasty.Taking traditional Chinese medicine internally and acupuncture externally can adjust both body and the mind.Professor JIA′s treatment of intractable insomnia based on liver and gallbladder theory was effective,which can provide the valuable experience for clinical treatment of intractable insomnia and is worthy of popularizing.
