黄连解毒汤加减治疗2型糖尿病的临床疗效

Curative Effect of Modified Huanglian Jiedu Decoction(黄连解毒汤)on Type 2 Diabetes Mellitus

目的观察黄连解毒汤加减治疗2型糖尿病患者的临床疗效及对血糖、肠道菌群的影响。方法选择医院2018年6月—2020年6月收治100例2型糖尿病患者,随机分为对照组、观察组,对照组采用西药常规治疗,观察组在对照组的基础上加用黄连解毒汤治疗。比较两组的治疗效果,治疗后中医证候积分改善情况、血糖控制情况及肠道菌群变化。结果观察组治疗后总有效率88.00%(44/50),显著高于对照组的总有效率72.00%(36/50)(P<0.05);两组治疗后中医证候积分显著低于同组治疗前,观察组治疗后中医证候积分显著低于对照组治疗后(P<0.05);两组治疗后空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)含量均显著低于同组治疗前,观察组治疗后FPG、2 hPG、HbA1c含量均显著低于对照组治疗后(P<0.05);两组治疗后革兰阳性杆菌(G^(+)b)、革兰阴性杆菌(G^(-)b)数量均显著高于同组治疗前(P<0.05),观察组治疗后G^(+)b、G^(-)b数量显著高于对照组(P<0.05),观察组治疗后球菌/杆菌(c/b)比值显著低于同组治疗前(P<0.05),两组治疗前后G+c、G-c数量差异无统计学意义(P>0.05)。结论在常规降糖治疗的基础上,给予黄连解毒汤治疗2型糖尿病,血糖控制效果良好,可调节肠道菌群数量。

Objective To observe the clinical curative effect of Modified Huanglian Jiedu Decoction on patients with type 2 diabetes mellitus(T2 dM)and its influences on blood glucose and intestinal floras.Methods A total of 100 patients with T2 dM admitted to the hospital were enrolled between June 2018 and June 2020.They were randomly divided into control group and observation group.The control group was treated with routine western medicine while the observation group was additionally treated with Modified Huanglian Jiedu Decoction.The curative effect,scores of TCM syndromes after treatment,blood glucose control and changes of intestinal floras were compared between the two groups.Results After treatment,total response rate of observation group was significantly higher than that of control group(88.00%vs 72.00%)(P<0.05).After treatment,scores of TCM syndromes in both groups were significantly decreased,which were significantly lower in the observation group than those of the control group(P<0.05).After treatment,levels of fasting plasma glucose(FPG),2h postprandial blood glucose(2 hPG)and glycosylated hemoglobin(HbA1c)in both groups were significantly decreased,which were significantly lower in the observation group than those of the control group(P<0.05).After treatment,the number of Gram-positive bacteria(G^(+)b)and Gram-negative bacteria(G^(-)b)in both groups was significantly increased,which was significantly more in the observation group than that of control group(P<0.05).After treatment,the ratio of cocci/bacillus(c/b)was significantly decreased in the observation group(P<0.05).There was no significant difference in the number of G+c or G-c between the two groups before and after treatment(P>0.05).Conclusion On basis of routine hypoglycemic therapy,Modified Huanglian Jiedu Decoction has good blood glucose control effect in the treatment of T2 dM,which can regulate the number of intestinal flora.
