加味生化汤联合米索前列醇治疗对稽留流产患者血清HCG水平、临床疗效及手术率的影响

Effects of Modified Shenghua Decoction(加味生化汤)Combined with Misoprostol on Serum HCG Level,Clinical Curative Effect and Surgical Rate of Missed Abortion Patients

目的探讨加味生化汤联合米索前列醇治疗对稽留流产患者血清人绒毛膜促性腺激素(HCG)水平、临床疗效及手术率的影响。方法选取2020年2月—2020年9月该院收治的稽留流产患者64例作为研究对象,随机分为米索前列醇组及联合组,每组32例。米索前列醇组给予米索前列醇治疗,联合组在此基础上加用加味生化汤治疗。比较两组临床疗效;比较两组完全流产患者临床指标;对两组完全流产患者治疗前后血清HCG、雌二醇(E_(2))水平进行比较;比较两组药流未完全流产及流产失败患者手术情况;对两组用药期间不良反应发生情况进行比较。结果联合组流产效果明显优于米索前列醇组(P<0.05);完全流产患者中联合组妊娠物排出时间、腹痛持续时间、阴道出血量明显低于米索前列醇组(P<0.05);完全流产患者中联合组治疗后相比米索前列醇组HCG、E_(2)水平明显降低(P<0.05);未完全流产及流产失败患者中联合组首次清宫术时间明显低于米索前列醇组(P<0.05);两组治疗期间不良反应发生率相比,差异无统计学意义(P>0.05)。结论加味生化汤联合米索前列醇治疗稽留流产可明显提高流产效果,改善患者凝血功能,减少阴道出血量,促进性激素水平恢复,缩短清宫时间,具有良好的应用价值。

Objective To explore the effects of Modified Shenghua Decoction(加味生化汤)combined with misoprostol on serum HCG level,clinical curative effect and surgical rate of patients with missed abortion.Methods A total of 64 missed abortion patients who were admitted to the hospital from February to September 2020 were enrolled as the research objects.They were randomly divided into misoprostol group and combination group,32 cases in each group.The misoprostol group was treated with misoprostol,while the combination group was treated with Modified Shenghua Decoction on this basis.The clinical curative effect,clinical indexes in patients with complete abortion,levels of serum human chorionic gonadotropin(HCG)and estradiol(E_(2))in patients with complete abortion before and after treatment,surgical conditions in patients with incomplete abortion and abortion failure and the occurrence of adverse reactions during medication were compared between the two groups.Results The abortion effect in the combination group was significantly better than that in the misoprostol group(P<0.05).The discharge time of pregnancy residues,duration of abdominal pain and vaginal blood loss of complete abortion patients in the combination group were significantly lower than those in the misoprostol group(P<0.05),and levels of HCG and E_(2)were significantly lower than those in the misoprostol group(P<0.05).The first curettage time of patients with incomplete abortion and abortion failure in the combination group was significantly lower than that in the misoprostol group(P<0.05).There was no significant difference in the incidence of adverse reactions during treatment between the two groups(P>0.05).Conclusion Modified Shenghua Decoction combined with misoprostol in the treatment of missed abortion can significantly improve the abortion effect and coagulation function,reduce vaginal blood loss,promote the recovery of sex hormones and shorten curettage time.
