黄芪建中汤加减治疗脾胃虚寒型胃溃疡的临床疗效

Clinical Effect of Modified Huangqi Jianzhong Decoction(黄芪建中汤加减)on Gastric Ulcer of Spleen-Stomach Deficiency Cold Type

目的探讨黄芪建中汤加减治疗脾胃虚寒型胃溃疡的临床效果观察。方法将2019年1月—2020年12月在该院消化内科治疗的126例脾胃虚寒型胃溃疡患者随机分为两组,对照组使用常规西药治疗,在此基础上,观察组使用黄芪建中汤加减治疗,对比两组的临床疗效、中医证候积分变化、血清因子水平变化、胃肠激素水平变化、不良反应。结果观察组治疗有效率为95.24%(60/63),明显高于对照组的77.78%(49/63)(P<0.05);观察组治疗后胃脘痛、喜温喜按、嗳气、反酸、饱胀、神疲倦怠等中医证候积分明显低于对照组(P<0.05);观察组治疗后白细胞介素-17(IL-17)水平明显低于对照组,转化生长因子-α(TGF-α)、表皮生长因子(EGF)水平明显高于对照组(P<0.05);观察组治疗后血清胃泌素明显低于对照组,血浆胃动素明显高于对照组(P<0.05);观察组恶心、呕吐、腹痛、腹泻、头晕等不良反应发生率明显低于对照组(P<0.05)。结论黄芪建中汤加减治疗脾胃虚寒型胃溃疡的临床效果显著,能明显减轻中医证候,降低机体炎症反应,改善胃肠动力,促进胃黏膜的修复,具有积极的临床意义。

Objective To investigate the clinical effect of Modified Huangqi Jianzhong Decoction(黄芪建中汤加减)on gastric ulcer of spleen-stomach deficiency cold type.Methods From January 2019 to December 2020,126 patients with gastric ulcer of spleen stomach deficiency cold type were randomly divided into two groups.The control group was treated with conventional western medicine,and the observation group was treated with Huangqi Jianzhong Decoction.The clinical efficacy,TCM syndrome score,serum factor level,gastrointestinal hormone level and adverse reactions of the two groups were compared.Results The effective rate of the observation group was 95.24%(60/63),which was significantly higher than 77.78%(49/63)of the control group(P<0.05).After treatment,the TCM syndrome scores of stomachache,warming and pressing,belching,acid regurgitation,fullness and mental fatigue of the observation group were significantly lower than those of the control group(P<0.05).The IL-17 level of the observation group was significantly lower than that of the control group,and the levels of TGF-αand EGF were significantly higher than those of the control group(P<0.05).After treatment,the IL-17 level of the observation group was significantly lower than that of the control group.After treatment,the serum gastrin of the observation group was significantly lower than that of the control group,and the plasma motilin was significantly higher than that of the control group(P<0.05).The incidence of adverse reactions such as nausea,vomiting,abdominal pain,diarrhea and dizziness of the observation group was significantly lower than that of the control group(P<0.05).Conclusion The clinical effect of Huangqi Jianzhong Decoction in the treatment of gastric ulcer of spleen-stomach deficiency cold type is significant,which can significantly reduce the TCM syndrome and the body′s inflammatory reaction,improve gastrointestinal motility,and promote the repair of gastric mucosa,which has positive clinical significance.
