小儿肺热咳喘颗粒治疗毛细支气管炎婴幼儿疗效分析

Analysis of Xiao′er Feire Kechuan Granule(小儿肺热咳喘颗粒)for 41 Cases of Infants with Bronchiolitis

目的观察小儿肺热咳喘颗粒治疗风热闭肺型毛细支气管炎婴幼儿的临床疗效及对患儿肺功能、炎症的影响。方法82例患儿随机分为对照组和观察组各41例,对照组予常规治疗,观察组加服小儿肺热咳喘颗粒,均治疗7 d。比较临床疗效、肺功能[潮气量(VT)、呼吸频率(RR)、潮气呼气峰流速(PTEF)、达峰时间比(Tptef/Te)、达峰容积比(Vpef/Ve)]、炎症指标[C反应蛋白(CRP)、白细胞介素(IL-6、IL-8)、肿瘤坏死因子α(TNF-α)],记录临床症状改善时间、住院时间和不良反应。结果观察组临床有效率为97.6%(40/41),高于对照组的80.5%(33/41)(P<0.05)。观察组发热、咳嗽、气喘及哮鸣音消失时间、住院时间均明显短于对照组(P<0.05)。治疗后,两组CRP、IL-6、IL-8、TNF-α含量及RR均明显降低,VT、Tptef/Te、Vpef/Ve明显增高(P<0.05)。组间相比,观察组IL-6、IL-8、TNF-α、Tptef/Te、Vpef/Ve及RR均明显优于对照组(P<0.05)。两组均无明显不良反应出现。结论小儿肺热咳喘颗粒治疗风热闭肺型毛细支气管炎婴幼儿安全有效,值得临床推广。

Objective To observe the clinical effects of Xiao′er Feire Kechuan Granule(小儿肺热咳喘颗粒)for infants with bronchiolitis(syndrome of wind-heat invading lung),and the impacts on pulmonary function and inflammation.Methods A total of 82 cases were randomly divided into control group(41 cases)and study group(41 cases).The control group received conventional treatment and the study group received Xiao′er Feire Kechuan Granule additionally.All patients were treated for 7 days.The clinical efficiency,pulmonary function(VT,RR,PTEF,Tptef/Te and Vpef/Ve),and inflammatory indexes(CRP,IL-6,IL-8 and TNF-α)were compared.The disappearing time of clinical symptoms and hospitalization time,and adverse reactions were recorded.Results The clinical efficiency was 97.6%(40/41),which was higher than that of control group(80.5%,33/41)(P<0.05).The disappearing time of fever,cough,asthma and wheezing rale and hospitalization time was shorter in the study group(P<0.05).After treatment,the levels of CRP,IL-6,IL-8 and TNF-αwere decreased,VT,Tptef/Te and Vpef/Ve increased in both groups(P<0.05).Compared with those in the control group,levels of IL-6,IL-8,TNF-α,Tptef/Te and Vpef/Ve were better than those in the study group(P<0.05).There was no obvious adverse reactions occurred in both groups.Conclusion Xiao′er Feire Kechuan Granule is effective and safe for infants with bronchiolitis(syndrome of wind-heat invading lung),and is worthy of widely clinical use.
