卫气营血辨证在放射性皮炎治疗中的应用

Application of Syndrome Differentiation of Wei-Qi-Ying-Blood in Treatment of Radioactive Dermatitis

放射治疗在恶性肿瘤的治疗中占据重要地位,约75%的患者需接受放射治疗。放射性皮炎是放疗过程中因放射线的照射而引起的一种皮肤黏膜炎症性损害,临床主要表现为毛发脱落、皮肤红肿、红斑、脱屑、疼痛、色素沉着、严重可见皮肤萎缩、腺体的损伤、皮肤溃烂出血,其发生的严重程度和患者的年龄、机体、照射剂量、照射种类、照射面积及机体自身的状态等原因有关,如不及时进行治疗会影响患者的生活质量,中断放疗计划,影响放疗进程。西医针对此放射性皮肤损伤,尚无标准治疗方案,而中医结合卫气营血理论将放射性皮炎进行辨证分型,分为邪犯肺卫证、热毒炽盛证、热毒伤阴证、阴损及阳证,又根据不同的证型阶段给予治疗用药,发挥中医药的优势,减轻患者的皮肤反应,降低皮炎的发生率,值得推广。

Radiotherapy occupies an important position in the treatment of malignant tumors.About 75%of patients need to receive radiotherapy.Radiodermatitis is an inflammatory damage to the skin and mucosa caused by radiation during radiotherapy.The main clinical manifestations are hair loss,skin redness,erythema,scaling,pain,pigmentation,serious visible skin atrophy,gland damage,skin ulceration and bleeding.The severity of its occurrence is related to the patients′age,body,radiation dose,type of radiation,exposure area and the body′s own state.If the treatment is not performed in time,it will affect the patients′quality of life,interrupt the radiotherapy plan and affect the radiotherapy process.Western medicine does not have a standard treatment plan for this radiation skin injury.Combined with the theory of Wei-Qi-Ying-Blood,traditional Chinese medicine divides radiation dermatitis into evil invading lung and Wei syndrome,heat toxin burning syndrome,heat toxin injuring Yin syndrome,Yin damage involving Yang syndrome.It is worthy of popularizing to give treatment according to different syndrome stages,give full play to the advantages of traditional Chinese medicine and reduce patients′skin reaction and the incidence of dermatitis.
