龙血竭促进肛周脓肿术后创面愈合的效果及机制

Effects and Mechanism of Longxuejie(Dragon′s Blood)on Wound Healing after Perianal Abscess Surgery

目的观察龙血竭对肛周脓肿术后创面愈合的作用,并探讨其对创面组织中表皮生长因子受体(EGFR)、血管内皮生长因子(VEGF)的影响。方法将入选的60例肛周脓肿患者采用随机数字表法分为对照组(n=30)和观察组(n=30)。两组均行肛周脓肿切开引流术,观察组术后采用龙血竭无菌油纱条换药,对照组行无菌油纱条换药。比较两组创面分泌物情况、创面愈合时间、创面组织EGFR及VEGF表达和创面愈合疗效。结果观察组和对照组的总有效率[96.67%(29/30)vs 86.69%(26/30)]比较,差异无统计学意义(P<0.05);术后1 d,两组创面分泌物评分比较差异无统计学意义(P>0.05),术后7 d、14 d观察组创面分泌物评分均低于对照组,差异有统计学意义(P<0.05);观察组创面愈合时间短于对照组,差异有统计学意义(P<0.05);术后7 d、14 d观察组创面组织EGFR、VEGF表达量均高于对照组,差异有统计学意义(P<0.05)。结论龙血竭能有效减少肛周脓肿术后创面分泌物,促进创面愈合,其机制可能为通过上调EGFR、VEGF表达促进创面毛细血管生成、加速表皮细胞迁移而加速表皮层修复。

Objective To observe the effects of Longxuejie(Dragon′s Blood)on wound healing after perianal abscess surgery and explore its influences on epidermal growth factor receptor(EGFR)and vascular endothelial growth factor(VEGF)in wound tissues.Methods Sixty patients with perianal abscess were divided into control group(n=30)and observation group(n=30)by random number table method.Both groups underwent perianal abscess incision and drainage.After surgery,the observation group was given dressing changes with Longxuejie(Dragon′s Blood)aseptic oil gauze,while the control group was given dressing changes with sterile oil gauze.Wound secretions,wound healing time,expressions of EGFR and VEGF in wound tissues,and curative effect on wound healing were compared between the two groups.Results There was no significant difference in the total response rate between the observation group and the control group[96.67%(29/30)vs 86.69%(26/30)](P<0.05).One day after surgery,there was no significant difference in score of wound secretion between the two groups(P>0.05).At 7 d and 14 d after surgery,the above score in the observation group was lower than that in the control group(P<0.05).The wound healing time in the observation group was shorter than that in the control group(P<0.05).At 7 d and 14 d after surgery,expression levels of EGFR and VEGF in wound tissues of the observation group were higher than those of the control group(P<0.05).Conclusion Longxuejie(Dragon′s Blood)can effectively reduce wound secretion after perianal abscess surgery,and promote wound healing.The mechanism may be promoting formation of wound capillary and accelerating migration of epidermal cells by up-regulating the expressions of EGFR and VEGF,and thus accelerating epidermal repair.
