从“脾胃内伤,百病由生”探讨溃疡性结肠炎病机观

Discussion on Pathogenesis of Ulcerative Colitis from Perspective of “Internal Injury of Spleen and Stomach Causing Diseases”

“脾胃内伤,百病由生”是《脾胃论》的核心观点,脾胃为后天之本,气血生化之源,脏腑之本,任何因素导致脾胃损伤,脾胃功能失调,失于健运,病由此生,脾胃被伤,为疾病发生的基础。溃疡性结肠炎病位在肠腑,脾胃与肠生理病理关系密切,其发病不离于脾胃内伤。基于“脾胃内伤,百病由生”观点,探析溃疡性结肠炎的发病机制,外感时邪、情志内伤、饮食不节、劳逸失度都可致脾胃内伤,或是素体脾胃虚弱,脾胃功能失健,无力纳运,导致水谷代谢失调,病理产物生成,气血化生不足,气机升降失调,肠腑功能失健,水湿积聚,酿蕴成浊,浊毒内蕴,壅溃成疡。文章则从“脾胃内伤,百病由生”观点出发,阐述溃疡性结肠炎发病与调理,并强调顾护补益脾胃在溃疡性结肠炎治疗中的重要性。

The theory of“the internal injury of spleen and stomach causing diseases”is the core view of Spleen and Stomach Theory.Spleen and stomach is the acquired foundation,the source of Qi and blood biochemistry and the foundation of viscera.Any factor leading to spleen and stomach injury and spleen and stomach dysfunction which results in diseases and spleen and stomach injury,is the basis of the occurrence of the disease.Ulcerative colitis is located in the intestine.Spleen and stomach is closely related to the intestine physiologically and pathologically,and its pathogenesis is not separated from the internal injury of spleen and stomach.Based on the viewpoint of“internal injury of spleen and stomach causing diseases”,this paper probed into the pathogenesis of ulcerative colitis.Exogenous pathogen,emotional injury,improper diet and improper of work and rest can cause internal injury of spleen and stomach.Or deficiency of spleen and stomach causes spleen and stomach failing to transport,resulting in metabolic imbalance of water and food,formation of pathological products,deficiency of Qi and blood,imbalance of Qi movement,loss of intestinal and viscera function,accumulation of water and dampness,resulting in turbid.From the point of view of“internal injury of spleen and stomach causing diseases”,this paper expounded the incidence and condition of ulcerative colitis,and emphasized the importance of protecting and tonifying spleen and stomach in the treatment of ulcerative colitis.
