冠心病并发射血分数保留的心力衰竭患者脉象时域特征与超声心动图指标的关联研究

Correlation between Time Domain Characteristics of Pulse and Echocardiography in Patients with Coronary Heart Disease Complicated with Heart Failure of Preserved Ejection Fraction

目的通过分析冠心病并发射血分数保留的心力衰竭(heart failure with preserved ejection fraction,HFPEF)患者脉象时域特征与心脏超声心动图指标的相关性,挖掘可以反映冠心病并发HFPEF病理状态的脉象时域特征。方法利用Z-BOXⅠ型脉象仪采集冠心病并发HFPEF患者脉图,运用时域分析法提取脉象时域特征,并采集患者超声心动图指标,包括左室射血分数(EF)、左心室短轴缩短率(FS)、心搏量(SV)、左室舒张末期容量(EDV)及左室收缩末期容量(ESV),采用典则相关分析法分析脉象时域特征与超声心动图指标之间的相关性。结果脉象时域特征与超声心动图指标的第一典则变量的相关系数为0.718(P<0.05);脉象时域特征H5/H1、W1/T与超声心动图指标SV在自身的第一典则载荷和在对方的第一典则交叉载荷均较大,表明SV的变化对脉象时域特征H5/H1、W1/T的影响较大,且SV与H5/H1成正相关,与W1/T成负相关。结论冠心病并发HFPEF患者脉象时域特征与超声心动图指标存在相关性,脉象时域特征H5/H1和W1/T一定程度上能反映SV。

Objective Our aim is to explore the correlation between the time domain characteristics of pulse and echocardiography in patients with coronary heart disease(CHD)complicated with heart failure of preserved ejection fraction(HFPEF).Methods The z-box I pulse meter was used to collect the pulse graph of patients with CHD complicated with HFPEF.The time domain characteristics of pulse were extracted by time domain analysis method,and the indexes of echocardiography were collected,including left ventricular ejection fraction(EF),left ventricular short axis shortening(FS),heart stroke volume(SV),left ventricular end diastolic volume(EDV)and left ventricular end systolic volume(ESV).The correlation between the time domain characteristics of pulse and echocardiography was analyzed.Results The correlation coefficient between the time domain characteristics of pulse and the first canonical variable of echocardiography index was 0.718(P<0.05).The first canonical load of pulse time-domain characteristics H5/H1,W1/T and SV of echocardiogram index in their own first canonical load and in each other′s first canonical load were large,which indicated that the change of SV had great influence on the time-domain characteristics of pulse.The influence of H5/H1 and W1/T was greater,and SV was positively correlated with H5/H1 and negatively correlated with W1/T.Conclusion There is a correlation between the time-domain characteristics of pulse and echocardiography in patients with CHD complicated with HFPEF,and the time-domain characteristics of pulse state H5/H1 and W1/T can reflect SV to a certain extent.
