江柏华以“疠气”分期辨治新型冠状病毒肺炎临床经验总结

Summary of JIANG Baihua′s Experience on Differentiating and Treating Corona Virus Disease 2019(COVID-19)by Stages with Epidemic Pathogens

新型冠状病毒肺炎(简称"新冠肺炎")是指由新型冠状病毒感染引起的肺炎。江柏华教授认为新冠肺炎病因为"疠气",病机总纲为"疠犯膜原、分传表里",治疗上主张以中医药进行分期、分阶段治疗。临床治疗初期疏利膜原、宣散透邪,以达原饮合藿香正气散加减;中期活血通络、清热祛湿、解毒散结,自拟银翘解毒汤加减;后期峻补元阳、开窍醒神,以参附汤合苏合香丸或安宫牛黄丸加减;恢复期健脾益气、养阴生津,以沙参麦冬汤合参苓白术散加减,临床上取得了良好疗效。该文总结江柏华教授辨治新冠肺炎临床经验,并附临床病案1则,以资借鉴。

2019-nCoV infections of the alveoli result in corona virus disease 2019(COVID-19).Professor JIANG Baihua participated in the treatment of COVID-19,using epidemic pathogens as the etiology,and using the pathogen in pleurodiaphrag⁃nzatic spaces and transmission between interior and exterior as the pathogenesis,treated by traditional Chinese medicine by pha⁃ses.In the early stage of clinical treatment,it is to open the pleurodiaphragmatic interspace and expel pathogenic factors,and add and subtract Dayuan Yin(达原饮)and Huoxiang Zhengqi Powder(藿香正气散).In the middle stage of clinical treatment,it should activate blood circulation to dissipate blood stasis,clear heat and expel damp,clear away toxic material and resolve hard lump,and add and subtract Yinqiao Jiedu Decoction(银翘解毒汤).In the late stage of clinical treatment,it is to restore Yang and eliminate phlegm for resuscitation,and add and subtract Shenfu Decoction(参附汤)and Suhexiang Pill(苏合香丸)and An⁃gong Niuhuang Pill(安宫牛黄丸).In the convalescent period,it is to strengthen the spleen and replenish Qi,nourish Yin and generate body fluid,and add and subtract the Shashen Maidong Decoction(沙参麦冬汤)and Shenling Baizhu Powder(参苓白术散),have produced good results.This article summarized Professor JIANG Baihua′s clinical experience in constitution differentia⁃tion and treatment variation of COVID-19,and using a clinical case for reference.
